General Summary

Performs a variety of pharmacy-related duties supporting the initiatives of WellSpan Population Health Services (WPHS) in the provision of value-based pharmaceutical care while supporting the ongoing success of the team through process development. This is a profoundly collaborative role serving as the expert in clinical pharmacy to the ambulatory care management team to assure the highest quality of care for value-based care populations. This role includes performance of a variety of reviews applying care management techniques to assist patients, caregivers, family members, physicians, hospital case managers, ambulatory care managers and ancillary service providers to achieve the patient's maximum functional potential.

Essential Functions:

  • Provides expert clinical pharmacy support to the ambulatory care management team, including but not limited to, medication reconciliation for complex patients, chronic disease management, transitions of care, value-based utilization reviews, medication adherence, and medication therapy management (MTM).

  • Engages with others in the care management team leveraging the expertise of a pharmacist to achieve department and system level goals and outcomes in total cost of care and quality.

  • Utilizes Motivational Interviewing techniques as a patient-centered approach to activate patients in self-management of their chronic conditions with the goal of improved symptom management and interruption of a negative disease trajectory.

  • Performs medication review and teach-back to ensure patient understanding and ability to adhere to medical regimens.

  • Partners with other WPHS staff to ensure customer service excellence in a patient and customer-centered model supporting WellSpan Health strategies in Patient and Employee Experience. This is evidenced through outstanding service to all customers, teamwork (Working as One), and fiscal responsibility through improvement and innovation.

  • Performs patient outreach according to established protocols documenting goals and interventions in the electronic medical record according to NCQA standards.

  • Participates in daily lean management huddles, identifies opportunities for improvement in workflows, clinical care management processes, and policy updates.

  • Provides expert clinical pharmacy support as needed to the utilization management team as they conduct reviews related to medications.

Qualifications

Minimum Education:

  • Doctorate Doctor of Pharmacy (PharmD) Required

Work Experience:

  • 2 years Inpatient or retail pharmacy experience working with patients and providers in the appropriate use of pharmaceuticals. Required

  • 1 year Care management or utilization management experience. Preferred

Licenses:

  • Licensed Registered Pharmacist Upon Hire Required

Courses and Training:

  • Residency in pharmacy. Upon Hire Preferred
