Assist qualified veterans in researching and applying for resources through the VA, and any federal, state, or local programs. Liaison between Veterans Inc and the VA or community medical clinics Work with the veteran\'s assigned multidisciplinary team to provide timely, appropriate veteran-centered care Work closely with the veteran and their families to assist them in communicating their preferences in care Develop and maintain positive, effective relationships with providers, physicians, family members, and clients
