White Plains Hospital Center Patient Materials Transporter in White Plains, New York

POSITION SUMMARY

The Patient Transportation Coordinator helps coordinate the movement of patients to and from the cancer center connecting patients to transportation resources available through Medicaid, Paratransit, community partners, grant funded transportation programs, and more. The Patient Transportation Coordinator will primarily be responsible for reviewing and coordinating non-emergent transportation requests working to connect patients to transportation support based upon various eligibility criteria. Chart review and/or calls to clarify transportation needs will be performed to assure appropriate modes and timing of transport. Responsible for communicating with various internal and external customers involved with patient movement. This position will also support some general administrative and scheduling responsibilities for interdisciplinary services at the cancer center. Adheres to all WPHC policies and procedures including dress code, code of conduct, customer service, attendance and submission of annual physical Collaborates and communicates with the health care team. Completes competency requirements. Performs all other related duties as assigned.

Essential Functions and Responsibilities Includes the Following:

. Understands and adheres to the WPH Performance Standards, Policies and Behaviors. . Works with Cancer Center, other hospital department staff and other community resources to become knowledgeable about available resources and services that may be helpful to patients. . Screens referred patients for transportation eligibility, documents screening and patient/caregiver interactions, and scans documents into EMR . Coordinates transportation to and from WPH CCC, other cancer testing sites and Montefiore health system sites. . Maintains relationships between transportation agencies/departments. . Investigates, reports, and escalates transportation complaints. . Processes invoices for transportation providers. . Collects and tracks data on volumes, etc. . Completes random customer satisfaction surveys after trip completions. . Assists in training other staff members performing this role and others involved in the process. . Develops, implements, and maintains effective and professional office practices and procedures. . Interfaces effectively with peers, patients, caregivers, and transportation customers and assumes responsibility for professional development of self and others per department policy. . Communicate with providers about unique patient needs, (i.e. cultural). . Performs all other related duties as assigned

EDUCATION & EXPERIENCE
