Community Care Home Health Services Home Health Aide in White Plains, New York

POSITION: Home Health Aide (HHA) General Purpose: A Home Health Aide (HHA) provides personal care and incidental / instrumental activities of daily living as assigned to patients / clients in the home setting as permitted by the policies and procedures of Community Care Home Health Services (Company) and any other federal, state , local law, or regulation. The Home Health Aide is under the direct supervision of the licensed RN. The HHA provides care in accordance with the DOH Matrix: Permissible and Non-Permissible Activities: HHA Services. Essential Functions: Assist, perform, and train patient / client and family in activities of daily living such as bathing, grooming, toileting, and adequate nutritional intake. Promote safe environment and maintain unobstructed pathways while providing care. Assist with ambulation, transfers, and / or range of motion exercises. Assist with incidental / instructional activities of daily living such as shopping, meal preparation, socialization activities, homemaking, maintaining a clean and safe / physical environment and reminding the patient / client to take their medication. Utilize infection control measures such as universal precautions, hand washing and personal protective equipment (PPE). Recognize, document, report changes in patient / client condition and / or safety to the supervisor / manager. Organize self to carry out visits / shifts and tasks. Attend mandatory Company In-Services and provide requested documents to keep employee files current Complete other assignments as requested and assigned. May have access to personal health information (PHI) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization / department. Minimum Education & Experience Requirements: Home Health Aide certification obtained by: o Successful completion of the Home Health Aide Training Program (HHATP) approved by NYS Department of Health or NYS Department of Education; or o Successful completion of approved Home Health Aide Competency Evaluation Program; or o Approved Personal Care Aide (PCA) Upgrade program. Knowledge, Skills & Abilities Required: Meets applicable health requirements to provide patient / client services. Able to effectively communicate with all levels of the workforce. Able to hear , speak, write, and read in a manner understood by most people. Able to read and interpret technical instructions related to providing services of the patient / client and use equipment required in providing services. Able to prioritize multiple tasks to effectively deal with multiple changes based on clients needs and scheduling. Working Conditions & Physical Effort: Work is normally performed in a residence, or senior living facility. Work may involve frequent exposure to communicable diseases, bloodborne pathogens and/or other potentially infectious or hazardous materials that require following safety precautions and using work practices that require the use of personal protective equipment. Able to travel from assignment to assignment within geographic area served. Medium physical activity that may require constant lifting, carrying, pushing, or pulling over 50lbs, and physically stretching, bending, stooping, twisting, standing, sitting, walking, reaching, and climbing stairs. Able to frequently lift, reposition, and transfer clients and assist with standing and walking. Able to consistently stand, bend, stoop, twist, stretch, squat, kneel, walk, climb stairs, and reach freely. Able to grasp with thump is opposition of fingers of palm, i.e manipulate pen, knobs, or objects. Ability to visually and auditorily observe the patient / client.
