Full (40 hours/week) or Part Time (20 hours/week) Position Available
Pay rate: $30-$33.50 per hour
Sign on bonus eligible
This position will work at our Rush-Scottsville Road home in Rush and may have to help out at our East River Road home in West Henrietta (the houses are 3 minutes apart)
Duties and Responsibilities
Interact regularly with each resident to develop rapport with them
Physically examine and determine health status for all residents on a regular basis and as needed
Monitor control of communicable diseases and infections
Accurately and consistently complete all required documentation practices utilizing the PrecisionCare software and in written form, when necessary (up-to-date health information files, Plans of Nursing Services (PONS), specific medical information, etc.)
Annually assess each resident’s ability to self-medicate in the form and format required by OPWDD and develop appropriate objectives in this area according to skill level
Aide in developing and maintaining all necessary medical, health and safety protocols and procedures as dictated by OPWDD regulations
Coordinate all medical/health related services (i.e., medical appointments), make service reports in the electronic medical charts and maintain effective communication with all health care providers for each resident
Review and record monthly doctors’ orders and MAR sheets (including controlled drug sheets, home med sheets and key transfer)
Ensure orders in EMAR (CareSuite) are accurate and clear for staff and complete weekly checks
Administer medically prescribed medications to the residents and follow up on all medication errors as dictated by Agency policy
Maintain medical cabinet with all medically approved medications, supplies and equipment as per OPWDD regulations
Maintain ongoing and effective communication with the nursing staff at each resident’s school/day program and accurately provide them with any records or medications needed
Help ensure that all medical, health and safety procedures are implemented as written
Monitor and maintain accurate data on staff required health testing mandated by the Department of Health
Attend appointments and all hospitalization discharges for residents or coordinate RN coverage when not available
Aide in providing teaching of the NYS OPWDD Medication Administration Course to appropriate residential staff including re-certifications on an annual basis
Oversee AMAPs and the DSP II Medical Liaison in medication administration responsibilities
Help schedule and provide in-service training sessions for residential staff as dictated by OPWDD regulations (detecting signs of illness or dysfunction that warrant medical or nursing intervention, first aid, etc.)
Maintain CPR and First Aid Instructor certification
Participate as an active member on the Residential Safety Committee and the Residential Interdisciplinary Team to establish, implement and maintain appropriate therapeutic programming for all residents
Attend all case reviews and provide written reports on the progress of each resident
Attend and participate in all team/program meetings and other meetings as requested
Review all Incident Reports and provide necessary medical attention and follow-up to injuries
Adhere to a rotating on-call schedule for triage and notification purposes (approximately one week per month)
Review and update the "On-Call Helpful Hints" document on a quarterly and as needed basis
All other duties as assigned by manager
Minimum Qualifications
Associate’s degree from a two-year college or university in Nursing
Certified by NYS as a Registered Nurse
Ability to read, write, speak and understand English
Possess a current and valid NYS driver’s license with an acceptable driving record
Be able to regularly lift and/or move up to 50 pounds
Preferred Qualifications
Bachelor’s degree from a four-year college or university in Nursing
One to two years related experience and/or training
Experience with individuals with developmental disabilities
Certifications and Trainings
Must maintain NYS RN Licensure
NYS Medication Administration and OPWDD Nursing Certification
SCIP-r
All other trainings mandated by the OPWDD
