Summary:
Under the supervision of a licensed pharmacist, the medication reconciliation technician performs multiple tasks related to collecting, reconciling, and entering the home medication history into the patient’s electronic health record prior to admission. Interviews patients and reviews medication-related documentation for the purpose of compiling a medication history. Compiled history information is entered into the patient’s electronic health record for reconciliation by the admitting provider. May perform a variety of functions which impact the medication reconciliation process and patient care.
Responsibilities:
· Collects an accurate medication list of the patient’s home medication in anticipation of hospital admission
· Gathers information for best possible medication list from various resources (patient interviews, physician offices, community pharmacies, historical encounters, extended care facilities, or other acute care facilities) to ensure a complete and accurate entry into the hospitals electronic health record.
· Effectively communicate with other healthcare professionals to prioritize patient care and identify patient specific needs
· Accurately enter home medications into the electronic health record for utilization by the provider responsible for admitting the patient
· Directs and unresolved issues to the admitting provider or supervising pharmacist
· Communicates delays or difficulties in the reconciliation process to the appropriate disciplines
· Restocks Omnicell automated dispensing machines
· Other duties as assigned
Education or Equivalent Experience:
H.S. Diploma/GED (Required)
Pharmacy technician certification preferred
2 years minimum with 1 year exp in retail pharmacy preferred
