SCHOOL BASED CLINICAL SOCIAL WORKER

JOB SUMMARY:

Provides Clinical social work services, including effective interventions, goal setting with clients, counseling and appropriate referrals to individuals, groups, and families. Works with staff of the Child & Adolescent Health Centers/Teen Health Centers/SWP/E3 mental health sites to accomplish goals and objectives of the center. Contributes to the departmental continuous quality improvement program.

ESSENTIAL DUTIES:

  1. Verifies patient identification and physician order.2. Provides procedural information/education to the patient/family prior to each exam.3. Acquires an accurate patient history and documents in the medical record.4. Performs Ultrasound and related patient care procedures using age-appropriate methods for patient’s physical ability and/or limitations.5. Proficiently operates ultrasound imaging equipment and performs necessary quality control and quality assurance testing.6. Monitors patients during procedures to ensure patient safety.7. Reviews ultrasound images with the Radiologist, as necessary.8. Ensures STAT requests are relayed to radiologist in a timely manner.9. When required, assists with procedures using sterile technique, receives and documents samples for lab processing. Disposes of fluids according to biohazard protocols.10. Ensures equipment is wiped down between patients and exam room is clean and properly stocked with linen and supplies.11. Accurately and efficiently uses hospital computer systems for verifying orders, viewing schedules, and sending information to PAC’s.12. Uses effective inter/intra-departmental communication and participates with medical staff, management, technical and support staff.13. Assist in training new staff/students.14. Verifies correct patient, correct exam / final verification, clarify questionable orders with ordering physician or Radiologist.

ADDENDUM TO ESSENTIAL DUTIES

  1. Demonstrates knowledge and commitment to the mission, philosophy, and objectives of the Beaumont CHAC/Teen Ctrs/SWP/E3 sites and the. Community Benefit/Health Division.

  2. In encounters with staff, patients, and their families, demonstrates Compassion, Respect, Integrity, Teamwork, Excellence.

  3. Provides ongoing, timely and regular clinical observation for: individual, group and family counseling

  4. Conducts timely psycho-social assessments and other professional documentation.

  5. When appropriate, works with school social workers, community probation officers and clinical staff to develop a plan of care for each client when appropriate.

  6. Assists staff in making appropriate client referrals to community and professional resources within and outside of the Beaumont system.

  7. Works in collaboration with schools, police, and other community agencies to develop and implement prevention and counseling programs.

  8. Provides clinical and professional consultation regarding teen center patients to other teen center staff

  9. Demonstrates knowledge of policy and procedures and regulatory compliance to optimize client care and safety.

  10. Supports performance improvement initiatives based on client satisfaction and quality

  11. Demonstrates respect and maintains client’s dignity and confidentiality.

  12. Participates in recruiting, orienting, and mentoring new staff.

  13. Maintains professional affiliations and memberships

  14. Demonstrates responsibility for own professional growth and development

  15. Uses EPIC for patient documentation

  16. Provides mental health training to teen center and school staff

  17. Provides mental health evidenced based curriculum to students

  18. Assists team members with grant reports

Education / Training:

  • Graduate of Diagnostic Ultrasound Program or equivalent.

    Work Experience :

  • Minimal as gained at student level.

    Certification, Licensure, Registration :

  • Registered by American Registry Diagnostic Medical Sonography (ARDMS) or Registry eligible, paid at the trainee rate for up to 1 year.

  • If unregistered must obtain at least one (ABD or OB/GYN) within 1 year from hire. Additional registry within two years from date of hire.

  • If registered at hire additional registry must be obtained within 1 year from hire.

    Other Qualifications :

  • Basic Life Support Certification for Healthcare Providers at hire through the American Heart Association.
