JOB REQUIREMENTS: Summary of Position Responsible for working with individuals to support them to obtain independent community living. This position will mentor and build relationships with individuals to assist them to connect to their communities and live more independently. Education and Experience High school diploma or equivalent. At least two (2) years of experience working with individuals with disabilities or in a mentor/teaching position that assists individuals in learning skills and providing them independently. Individual demonstrates experience and understanding of community resources and methods of teaching independent living skills. A valid driver\'s license with good driving record and insured vehicle available for daily use. This position requires annual background checks. Knowledge, Skills and Abilities/Essential Functions **Following is a listing of representative examples of duties for this position; it is not considered to be a complete listing of all duties and responsibilities for the position. Maintain a professional image while working in the community and in individuals place of residence. Assist individuals with any activities of daily living that are needed to live independently. This can include but not limited to cleaning, cooking, laundry, medication support, bill paying, and personal care support. Develop community relationships that connections can be made between the person supported and community organizations (volunteer, or join a club as example). Make decisions and work independently. Build strong relationships with the individuals that you work with to assist in learning and obtaining new skills that support continued movement toward more independence. This position does not provide a support (example cook for someone) but instead teaching for the purpose of the individual gaining independence. Communicate effectively in person and for record keeping. Work cooperatively with all people. Work a flexible schedule some evening hours and weekend work. Will be required to be on call 24/7 on a rotating schedule within the team. Document outcomes and action steps for each individual supported. Break tasks down to achievable components. Responsible to schedule and complete work assigned for the position. Maintain working relationship with community partners to assist with community connections for individuals we support. Perform creative problem solving as needs arise for individual in the community and in their home/apartment Work cooperatively Community Support team and manager to develop and update outcomes and action steps for successful support. Work one-on-one with individuals or in small groups on a daily basis. Implement personal profile plans on a daily/weekly basis to meet the individuals outcomes. Willingness to complete first aid and CPR training and certification if requested to. Case management III. Physical Requirements Ability to facilitate, initiate and respond to verbal, written, computerized, telephone and fax communications. Ability to demonstrate, train and/or model job tasks/duties/responsibilities. Ability to perform quality inspection according to ODC\'s and/or customer\'s requirements. Mobility to access various locations within/outside ODC facilities. Physical ability to lift or transfer adults when providing personal care. Physical ability to provide and/or respond to behavior intervention. Lifting 25 35 lbs frequently up to 60 lbs occasionally.
