Position Type/Expected Hours Of Work - Flexible to the needs of the program (typically Monday-Friday; 7:30am-4:00pm)

Qualifications - Associates Degree with two years’ experience working with developmental disabilities required. Valid New York State Driver’s License. Must have knowledge of OPWDD and Medicaid regulations. Must be organized, detail oriented and able to work in a fast paced environment.

Work Authorization/Security Clearance - Authorized to work in the United States and pass all background clearances.

Summary/Objective -Supervision of Community Habilitation staff supporting a person’s daily program, stressing independence, inclusion and individualization with emphasis on the person’s choice. A person centered philosophy will be maintained. Oversight of the Fiscal Intermediary and Self-Direction support programs. Must have knowledge of OPWDD and Medicaid regulations. Must be organized, detail oriented and able to work in a fast paced environment.

  • Responsibly implement and manage your approved Self-Directed Budget caseload. This includes collaborating with your Circle of support, Care Manager and Self-Directed Liaison.

  • Preparing/reviewing Monthly Expenditure Reports and work with your Circle of Support and Support Broker to use these reports to maintain spending within your budget.

  • Support positive, collaborative relationships with colleagues, service organizations and community members.

  • Work collectively with the Community Resource Professional on all recruitment opportunities.

  • Administrator over web based system for tracking program participant and program operations.

  • Conduct in field mentoring sessions with the DSPs who are providing the Community Habilitation services.

  • Attend Circle of Support meetings twice a year or as needed for people participating in Self-Direction.

  • Ensure documentation is accurate and prepared per OPWDD regulations for billing for both the Community Habilitation program and the Self-Direction/Fiscal Intermediary program.

  • Responsible for assisting in the training and supervising of Direct Support Professional/Substitute to ensure staff is competent to provide rehabilitative services and meet the needs of the People We Support within the context of the regulation.

  • Responsible for training people supported and family members on documentation submission into the web based system.

  • Observe and provide feedback on best practices to adhere to the Code of Ethics and Core Competencies while providing person centered support.

  • Conduct 90 day performance evaluations and annual performance evaluations for DSPs for Community Habilitation staff and encourage the completion of performance evaluations by the family for self-directed staff.

  • Conduct bi-monthly site visits to DSPs in the field and assist them in effectively delivering the persons support based Community Habilitation plan.

  • Effectively support DSPs in trouble shooting and work related issues/concerns.

  • Demonstrate flexibility, accuracy, and confidentiality in work assignments.

  • Maintain confidentiality of all work related information.

  • Shall attend and demonstrate proficiency by written examination on all required in-services by achieving 80% competency on the exams, noted through OPWDD compliance.

  • Shall report any illness or accident, changes, or problems in a person’s medical status to the nurse and shall use the appropriate documentation procedure and follow instructions as given.

  • Shall write incident reports as defined by Incident Review Procedures.

  • Communicate all matters concerning the People We Support to Circle of support team and direct supervisor.

  • Agree to abide by the Code of Conduct for Service Delivery by reviewing and signing the document initially and as changes are made.

    Supervisory Responsibility - None

    Travel - Based on the needs of the program within Jefferson and St. Lawrence counties.

    Work Environment - This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
