Description
San Mateo County Health - Family Health Services is seeking two Public Health Nurses (PHN) -Limited Term to support any of the Child Health Programs, including the Pediatric Program. The Pediatric Program is a muti-disciplinary team and incorporates an evidence-based home visiting model that has been demonstrated to significantly improve the relationship between parents and their young children (0-5).
The Pediatric PHN home visitors are responsible for providing comprehensive nursing services to children and their families using an evidenced-based home visiting program. The Nurses will work to promote the health of families and their young children, strengthen parent-child relationships and support a healthy environment for the entire family. The PHNs will make referrals to community and other agencies, coordinate care with hospitals and medical providers, educate and counsel parents. The Nurse home visitors are responsible for maintaining the highest standards in clinical nursing practice and adherence to policies, procedures, program visit guidelines, and standards.
While the primary assignment will be in the Pediatric Program, successful hires may support the Family Health Services Childhood Lead Poisoning Prevention Program and may be required to support other Family Health Services programs and activities based on departmental needs.
The Pediatric PHN home visitors will provide home visits and virtual visits to eligible families from birth through the child’s 5th birthday. They will carry a caseload of 25 or more clients including:
Bi-weekly to weekly home visits, and as needed.
Case management and care coordination (the goal is to coach clients to confidently advocate for themselves and their child over time).
Promotion of positive family health practices through nursing education.
Apply Trauma Informed Care / Adverse Childhood Experiences during visits with families.
Support of Parent/Child dyad through the use of relationship-based child development curriculum and activities.
Incorporate a strengths-based approach to develop therapeutic relationships with families.
Apply motivational interviewing concepts to support behavioral change.
Timely documentation in multiple EHR systems for nursing practice and data collection for evidence-based programming.
Participate in team meetings and case conferences as well as “Reflective Supervision” and 1:1 meetings with Public Health Nurse Supervisor.
Adhere to the nursing process as indicated by the CA Board of Registered Nursing.
Currently, there are two limited-term, full-time vacancies.
The ideal candidates would possess the following:
Proven ability to establish interpersonal relationships with individuals and families from diverse socioeconomic and cultural backgrounds.
Strong background in normal child growth and development and parent-child relationships with children from birth to 5 years old.
The ability to engage families from a family-centered, strengths-based model.
Demonstrates the ability to learn and practice supportive parenting and relationship promotion between parent and child.
Knowledge of trauma-informed care and adverse childhood experiences.
The ability to perform health and developmental assessments.
Excellent case management skills.
Excellent written and verbal communication skills.
Basic computer skills, electronic health record experience preferred.
High degree of autonomy. Ability to impose structure throughout the workday, both in between and during visits.
Understand, support, and coach others in the Family Health Mission of joining with families as early as possible to meet their goals towards better health and a brighter future.
Assist in creating a positive work environment that promotes mentoring, teamwork, and collaboration.
Fluent in Spanish (ability to speak, read and write) is required.
This position requires possession of a Class C California driver license or the ability to provide suitable transportation (including public transportation), as approved by the appointing authority.
NOTE: These are limited-term, at-will positions. Limited Term Employees are assigned to assignments not to exceed three years. They receive medical and dental benefits, similar to regular employees, and a defined contribution retirement plan.
Examples Of Duties
Duties may include, but are not limited to, the following:
Assess client's health status through history taking, observation and screening tests.
Develop care plan including goals and time frames.
Counsel, teach, and coordinate health and social services provided to client/family.
Inform high risk group of resources and make referrals as appropriate.
Document services and client's response to services.
Provide limited therapeutic nursing care; administer medication and treatments, teaches self-care when appropriate.
Participate in defining and addressing community health needs through consultation, recruitment of providers and development of resources.
Participate in organizing clinics, staffs and manages clinics; perform physical assessments, screening tests.
Participate in developing instruction materials.
Keep statistical records, write reports.
Assist with orientation instruction and guidance of community workers, and related personnel.
Perform related duties as assigned.
Qualifications
Education and Experience:
- Any combination of education and experience that would likely provide the required knowledge, skills and abilities is qualifying.
Licensure/Certification:
California license as a Registered Nurse.
California Certificate as a Public Health Nurse.
Knowledge of:
Principles and practices of public health nursing, with emphasis on field and clinic activities.
Problem solving through nursing process.
Technical nursing procedures.
Health and social services resources.
Collaborative health planning with multi-disciplinary teams.
Skill/Ability to:
Teach and counsel.
Perform technical nursing procedures.
Assess physical, psycho-social and nutritional needs.
Assess community health needs.
Teach and guide other health personnel and volunteers.
