Job Overview:

In this role, theCase Management Supervisorprovides oversight of CM activities on the weekends and requires a schedule inclusive of weekends each week.

Qualifications:

Required:

  • Minimum 3 years clinical experience in hospital, home health, or community setting

  • Minimum 2 years experience in acute care case management

  • Minimum 1 year supervisory experience

Preferred:

  • If RN, BSN required, and master s degree in Nursing, Social Work, Healthcare Management, or comparable field preferred

  • If Social Worker, MSW required; LCSW preferred

Certifications and Licenses Required

  • Current and valid license to practice as a Registered Nurse OR Licensed Social Worker in the state of New Jersey

  • Certification in Case Management (ACM or CCM) strongly preferred/required within 1 year of hire

Schedule:

Full time 40 hours to include every weekend Saturday and Sunday.

Essential Functions:

This is a full-time position responsible for the oversight of CM activities on the weekends and requires a schedule inclusive of weekends each week

Responsible for planning, managing, implementing, and evaluating the daily operations of the Department of Case Management which includes oversight of effective resources utilization and the provision of high quality and excellent care

Work in collaboration with CM Director, administration and physician leadership to establish measurable standards and outcomes

Oversee nurse case management and social work services

Provide education to the frontline CM team; Conduct quality monitoring of staff productivity and documentation to ensure compliance with regulatory requirements; Conduct reviews and follow up of patient experience concerns to ensure high quality of care; Ensure continued development of the program to improve site performance of the CM Department and to enhance provision of case management services
