Coastal Health and Wellness is seeking a Home Based Care Licensed Vocational Nurse (LVN)!

The Home Based Care Licensed Vocational Nurse position is responsible for providing direct patient in-home care under the supervision of a medical doctor or registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Required:

  • Graduation from an accredited nursing program.
  • Licensed to practice as an LVN in the State of Texas.
  • 2+ years of clinical experience.
  • Must have knowledge of adult and child learning principles, ambulatory nursing practices and procedures, and community resources.
  • Bilingual in English/Spanish a plus.
  • Current CPR Certification.
  • Ability for spur of the moment transportation without relying on organization vehicles.
  • Must possess or ability to readily obtain a valid drivers license issued by the State of Texas for the type of vehicle or equipment operated.
  • Department of Motor Vehicle check may be required, if applicable.
  • Customer Service Oriented.
  • Excellent written and verbal communication skills.
  • Excellent Computer Skills MS Office.
  • Must be willing and able to work evening and weekend hours if necessary.
  • Must be in compliance with GCHD Immunizations policy.
  • Must be in compliance with ICS training requirements.
  • Must pass criminal background check and drug/alcohol screening.

An equivalent combination of education and work experience which appropriately demonstrates the knowledge, skills and abilities to perform the above-described essential functions will be considered when hiring for this role.

Traditional Duties:

  • Conduct thorough in-home health assessments to evaluate patients' vital signs, medication adherence, and overall health status.
  • Develop individualized care plans in collaboration with the healthcare team, meeting patients' needs and goals.
  • Administer medications, treatments, wound care, and medical procedures following prescribed protocols and care plans.
  • Monitor and document patient condition changes, promptly communicating updates to the healthcare team.
  • Educate patients and their families on disease management, medication adherence, and lifestyle adjustments to promote wellness.
  • Collaborate with various healthcare professionals to ensure a holistic approach to patient care, including physical therapists, occupational therapists, and social workers.
  • Perform Nurse Only visits.
  • Performs prior authorizations.
  • Assist with supply inventory.
  • Completes patient forms when necessary.
  • Provides reports to other departments.
  • Performs medications disposal for expired medications.
  • Assesses patient education readiness and modifies patient education according to patient and family needs.
  • May serve on clinical teams.
  • Understands and follows GCHD/CHW Infection Control policies, plans, procedures and guidelines related to program area requirements.
  • Properly disposes of biohazard waste.
  • Adhere to job specific competencies within established departmental deadlines.
  • Follows standing delegation orders.
  • Performs end of shift Nurse Report.
  • Reports any problems to the Nurse Supervisor or appropriate manager.
  • Receives and follows up with High Priority lab results.
  • Responds to emergency codes.
  • Ensures optimum clinic flow by observing and addressing Nurses, MA , and Patients needs when applicable.
  • Communicating with upper management regarding Nurse/MA performance, staff issues and changes to treatment protocols.
  • Remaining calm under pressure and be able to delegate staff responsibilities quickly and effectively during busy shifts or emergency situations.
  • In-depth understanding of facility policies, procedures, and protocols.
  • Performs other duties as assigned by supervisor.
