Baylor Scott & White Health Sepsis Coordinator in Temple, Texas
JOB SUMMARY
The Sepsis Coordinator collaborates with the clinical team, physician partners and leadership to integrate the best practices and data analysis to provide quality care of the sepsis patient population. The coordinator facilitates evidence-based practice in accordance with current sepsis guidelines, contributing to positive patient outcomes and striving for excellence.
ESSENTIAL FUNCTIONS OF THE ROLE
Acts with integrity, caring, and respect while effectively facilitating teamwork to accomplish tasks and meet performance goals.
Communicates timely, professionally and works cooperatively with others.
Stays current on all Sepsis regulatory measures and Value Based Purchasing (VBP)
Interprets and presents data to facility/BSW committees and key stakeholders.
Communicates opportunities for improvement with team members, including providers, nursing, lab personnel, pharmacy and other caregivers who may benefit from the information.
Acts as liaison between clinical staff & Informatics/IS team to disseminate information & trouble shoot problems during transition of new implementation of clinical documentation in EPIC.
Provides education to to advance and enhance knowledge of the sepsis patient and treatments along the continuum of care from infection to recovery.
Facilitates participation in Sepsis awareness activities both at the system and the local level.
KEY SUCCESS FACTORS
Proficient use and understanding of EPIC
Proficient in MS word, excel and power point
Leadership experience is preferred

EDUCATION - Bachelor's or 4 years of work experience above the minimum qualification
