ProMedica Health System RN Navigator - Burns and Antoniolli Oncology - Full Time in Sylvania, Ohio

POSITION SUMMARY

The role of the Cancer Care Navigator is to enhance patient empowerment and facilitate continuity of care for patients with cancer. The Cancer Care Navigator will assist patients, families, and caregivers to overcome healthcare system barriers, and provide education and resources to facilitate well-informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer care continuum.

ACCOUNTABILITIES

  1. Review and evaluate distress and barrier screenings, matching unmet needs with appropriate services. Identify potential and realized barriers to care and facilitate appropriate referrals to alleviate barriers.

  2. Provides evidence-based education on diagnosis, facilitates communication between patient and various treatment teams.

  3. Facilitate in clarifying treatment goals, survivorship planning and identify end-of-life concerns. Provides education to patient regarding plan of care and palliative/end of life concerns.

  4. Facilitates EMR charting to ensure open communication between the interdisciplinary departments and patient.

  5. Monitors for positive pathology and facilitates scheduling of appointments for plan of care.

  6. Participates in tumor board meetings and other multidisciplinary meetings. Collaborate with peers in area of specialization in order to increase knowledge base and bring new ideas to professional practice.

  7. Establishes positive relationship with cancer patients, families, supports physicians, referring physicians, and other providers.

  8. Participate in quality improvement and strategic initiatives. Collects data for quality and performance improvement measures. Participate in quality improvement measures to ensure decreasing disparity of care.

  9. Empower patients with education in maintaining compliance with treatment.

  10. Collaborates with staff, philanthropy, community outreach, and community education.

Education: Associate degree in Nursing. Must obtain BSN within 5 years.

Skills: A strong background in oncology, including NCCN guidelines, treatment regimens, and educational resources. Must have strong leadership skills, critical thinking abilities, and the ability to work independently. Highly knowledgeable of the environment and system through which the patient must move in order to obtain care in addition to community resources and programs Culturally attuned to the people of the community being served Excellent communication skills and excellent organizational and ability to prioritize Flexible, sensitive, and compassionate

Years of Experience: 5+ years of experience in nursing

License: Registered Nurse in state of assigned practice

Certification: BLS, OCN or must obtain within 2 years
