JOB REQUIREMENTS: The focus of the Service Facilitators is to ensure that each client is being empowered and supported so he/she can achieve the highest possible level of independence and functioning. They serve as members of the recovery team and encourage the team to consider community resources that have appeal to the client before relying on professional services. The Service Facilitators explain and teach clients about their rights and make sure they understand the options of using formal and informal grievance resolution processes. They provide direct assistance and support to clients. They check in with participants on scheduled and emergency bases to make sure that service plans are being followed and continue to be useful in meeting their life goals. The function of the Service Facilitator is to assist clients in using their skills, so they are truly directing their own service plans. They are responsible for screening and assessment, coordination of services, monitoring services and clients progress, completing recovery plans, discharges, and documentation.
