Summary: Provide housing navigation & case management services to participants co-enrolled in SSVF (Supportive Services for Veteran Families) & VA Medical Center, HCHV (Health Care for Homeless Veterans) Outreach Center. Work with referred veterans & their families to assess housing resources, needs, barriers, & make an appropriate housing plan cooperatively with the Veteran & execute plans toward housing stability & permanence. Provide ongoing case management to participants with pending HUD (Housing & Urban Development) -VASH (Veterans Affairs Supportive Housing) housing vouchers. Position requires the ability to drive for company business which includes a valid driver's license, clean driving record, & proof of personal auto insurance. Must pass driver record & background check.

Education &/or Experience: Bachelor's degree from four-year college/university; or one to two years related experience &/or training; or equivalent combination of education/experience.

Essential Duties & Responsibilities include the following:

Work closely with the VA medical center's homeless programs & residential & community care programs to ensure that coordination is facilitated & veterans' direct care, referral, & follow-up are contiguous & based on a continuum of services.

Ensure program criteria, referrals, & procedures are maintained for HUD-VASH, SSVF, & program Veteran participants.

Develop working relationships & agreements with other organizations & directly coordinate with the designated PHA regarding housing voucher application & award process for each Veteran. Responsible for reviewing the agreements between Property Managers, PHAs, & the VA to establish appropriate referral sites for placement.

Establish & document participant eligibility. Gather & record information about the participant to identify & coordinate those services that will be most effective & acceptable to the participant improving housing stability.

Responsible for data collection, administrative record keeping, & performance measurements of participants & that everything is current, accurate, & in compliance.

Regularly conduct & participate in outreach activities, including field interviews, assessments, & referrals for homeless Veterans.

With a cooperative attitude, provide services in accordance with SSVF & VA Medical Center, HCHV Outreach Center, Goodwill policies, CARF standards, & principles of safety to implement the organization's missions & goals.

Facilitate Person-Centered Planning to determine vocational goals, recommend appropriate services to achieve goals, & oversee the program & objectives agreed upon.

Locate landlords willing to lease to clients with housing barriers.

Understand State & Federal Landlord-Tenant laws.

Determine rental & deposit assistance that might be beneficial to program participants.

Advocate for the Veteran with community resources, provide community referrals & mediation.

Consistently communicate & treat everyone in a courteous, tactful, & respectful manner.
