Providence Health & Services Diabetes Educator in Sherwood, Oregon


Providence is calling a Diabetes Educator (1.0 FTE, Days) to our Providence Medical Group Clinics.

This full time benefits eligible position will primarily support within our Progress Ridge and Sherwood clinic locations which offers a unique opportunity to work with patients throughout all different life stages (peds, adults and women experiencing gestational diabetes).

In this position, you will:

  • Provide education for clients with diabetes of all ages according to the individualized needs of the client.

  • Follow a process that involves a health and education assessment, development, implementation and evaluation of an education plan, while taking into account psycho-social, economic, and language barriers.

  • Perform your duties in a classroom or individual outpatient setting in collaboration with a team of Registered Nurses (RN's) & Registered Dietitians (RD's).

  • Accept professional development and community outreach as an integral part of this professional practice.


Required qualifications for this position include:

  • Registered Nurse - or - Registered Dietitian.

  • Current Oregon licensure

  • 3 years of experience including experience in treatment and education of diabetes within the last 2 years

Preferred qualifications for this position include:

  • Certification in Diabetes education and/or willingness to attain CDE certification

  • Formal course work or credentials in principles of adult learning theory and patient-centered learning

  • Bilingual in Spanish - or - Russian - or - Vietnamese and experience with diverse ethnic groups.
