Kaiser Permanente LICSW Clinical Practice Specialist (HH/HS) in Seattle, Washington

WORK WITH INTERDISCIPLINARY GROUP OF LEADERS TO PROVIDE EDUCATION, CLINICAL SUPPORT TO PROGRAM. MANAGEMENT EXPERIENCE PREFERRED

Accountable for education, program development and implementation to support knowledge and skills of clinical staff; develops training to support key initiatives in quality/redesign/practice improvement; accountable for clinical orientation, extended orientation of new LPN, RN & HHA as well as MSW and rehab staff in Home Health and Hospice. Supports implementation of strategies to recruit and retain clinical staff in Home Health & Hospice; provides consultation and leadership to clinical teams in the orientation, training, and selection of new employees; in collaboration with assigned Team Manager and Associate Director of Home Health & Hospice implements strategies to achieve, monitor, and evaluate clinical, service and regulatory targets; supports team manager in achieving desired performance of clinical staff.

Essential Responsibilities:

  • Develop, implement, & evaluate education programs to support knowledge and skill development of clinical staff working in Home Health and Hospice. Assure orientation programs for new clinical staff including extended orientation for RN, LPN, MSW, PT, OT, SPstaff

  • Develop on site training to support key clinical initiatives in quality/redesign/practice improvement

  • Provide consultation to Home Health and Hospice clinical practice teams and team managers in the orientation, training, and selection of new employees

  • Provide consultation to clinical staff staff in assessing patient, family age-specific clinical needs and contribute to the development of care plan for complex, high-risk patients. Provide leadership to evaluation of the effectiveness of care. Make recommendations to change standards of care or standards of practice based on research. Define and implement best clinical practices. Provide consultation to intrerdisciplinary practice team in meeting care needs of individual or groups of patients in Home Health and/or Hospice settings.

  • Support team manager in achieving desired performance of clinical staff

Basic Qualifications:

Experience

  • Minimum two (2) years of prior home health/hospice experience

Education

  • Graduate from an accredited school of nursing, master of social work, or physical therapy.

License, Certification, Registration

  • Washington State RN license, LICSW, or PT: WA PT license based on level of education required by the date of hire/transfer.

Additional Requirements:

  • Clinical expertise in team & program development; verbal and written communication; clinical practice in home based setting, teaching; group facilitation; program evaluation; individual and group skills assessment; adult learning theory & practice required computer skills required.

Preferred Qualifications:

  • Two (2) years of prior home health/hospice clinical management or clinical specialist experience preferred.

  • Master's degree in nursing or therapy.
