FULL-TIME Nonexempt : 40 hours/week

Coordinated Care Services Inc (CCSI) (https://positions.paylocity.com/PositionManagement/wizard/2033539) seeks a talentedRapid Engagement Delivery (RED) and Service Access Coordinatorto contribute their expertise in providing essential business services to our provider organizations and customers. The Rapid Engagement Delivery (RED) and Service Access Coordinator provides the support and guidance needed to engage and then connect high risk/high need individuals to services as rapidly as possible, including but not limited to, services such as DSS, Social Security, treatment, rehabilitative, recovery, residential and other community-based resources. Assesses eligibility for financial assistance and facilitates enrollment for those eligible using the RED team process. Assists with Mental Health SPOA, CD SPOA and homeless outreach efforts as requested.

CCSI is partnering withMonroe County Office of Mental Healthto hireaRapid Engagement Delivery (RED) and Service Access Coordinator.This position is employed by CCSI and working on behalf ofMonroe County.

In this role, you will be

  • Assess eligibility for DHS temporary assistance and then assist clients with obtaining assistance

  • Gauge client level of need and makes referral to appropriate community providers as rapidly as possible

  • Work with DSS financial assistance and emergency housing unit

  • Provide care management services to clients, employing person centered, motivational and other evidence-based practices

  • Initiate and follows up on referrals, paperwork and telephone contacts needed for enrollee’s immediate access to behavioral health and social services

  • Document, enter and maintain records of enrollments, dispositions, team meetings, trainings, etc. as appropriate

  • Conduct routine outreach to homeless shelters, client homes, clinics and other appropriate venues

  • Attend community meetings, staff meetings and training related to job duties

  • Other duties as assigned.

Requirements

Education and Experience

  • Bachelor’s in mental health, social work, or related human services field or equivalent education and work experience (see CCSI's equivalency chart (https://www.ccsi.org/CCSI/media/pdfs/Equivalency-chart.pdf) )

  • Experience with a Windows based operating system (Word processing, spreadsheet and database management software

Additional Requirements

  • Valid Class D New York State Driver License and access to a motor vehicle to meet transportation requirements of position

  • Ability to satisfactorily complete required background screenings
