Description

Essential Duties and Responsibilities

Monitors and coordinates client enrollment.

  • Tracks enrollments for program.

  • Works with staff to ensure that funder enrollment requirements are being met. Communicates directly with funders to respond to questions or issues related to the program.

  • Develops a plan for reaching intake and service milestone objectives.

  • Responsible for enrollment numbers reporting to be provided to supervisor.

  • Conducts outreach within the community to market services to potential clients.

  • Advocates on behalf of clients by referring them to appropriate community services or case management.

  • Monitor client cases to assist clients in attaining long-term economic self-sufficiency. Identifies obstacles to success and identifies potential solutions.

Maintains accurate records and files.

  • Reconciles reports and prepares adjustments, review bills and invoices for accuracy.

  • Plans and organizes work to ensure that state-mandated deadlines are met while maintaining all files for clients in strict accordance with Agency, department, regulatory and funder guidelines.

  • Communicates and partners with the department and agency personnel as required to ensure on-going and effective program operations.

  • Is aware of and actively supports the Agency’s Core Competence initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse.

  • Actively supports the financial stability of the department by reconciling reports, preparing adjustments, reviewing bills and invoices for accuracy while meeting or exceeding established productivity and documentation standards.

  • Assists with executing programmatic plans and contributing to program development.

  • Adheres to all applicable federal and state laws, including but not limited to those governing client confidentiality, privacy, program standards and billing and documentation standards.

  • May be crossed trained in other functions to support departmental needs.

  • Provides back-up clerical support to all department management staff.

  • Maintains up-to-date professional knowledge through reading, workshops/training, and conferences, as appropriate.

  • Participates in staff and department meetings as required.

  • Awareness of and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse

  • Other responsibilities as requested by supervisor

Qualifications

Education: Bachelor’s or master’s degree in Human Services, Social Work, Business Administration or related field preferred. Equivalent combination of education and experience will be considered

Credentials: Valid and clean NYS Driver’s License.

Experience: Minimum one year of experience in human services or related field. Experience working in a multi-cultural environment is preferred. Proficiency with MS Word, Excel desired.
