Compensation Range:

$20.99 - $28.34

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

GENERAL PURPOSE

Ensures patients receive the best care available to them by guiding patients through all the various complexities of the healthcare system. Provides field-based case management services. Serves as the main point of contact for clients. Builds strong relationships with clients in order to help clients stay engaged in medical care and adhere to their care plan. Identifies critical resources, helps navigate through health care services and systems, and promotes client health. Works closely with the care team, which may include doctors, nurses, and other clinical staff to support positive client health outcomes. Responsible for educating patients about their rights, insurance coverage, and payment options, as well as addressing any queries or complaints.

ESSENTIAL FUNCTIONS

  • Responsible for comprehensively supporting the coordination of care. Establishes close relationships with and serves as primary point of contact for clients. Communicates with care team members, including Care Coordinators, primary care physicians and other health care providers, to facilitate client care. Identifies resources for clients to overcome barriers to care, such as transportation, housing, and childcare arrangements. Acts as an advocate and liaison for participants/families and community-based organizations/services, provides referrals as appropriate. Provides health-related education, promotion, and support. Conducts phone calls for verification/confirmation, insurance medication formulary, pharmacy benefits clarification, and prior authorizations. Assists with general problem-solving as needed.

  • Maintains documentation of all client encounters and completes reporting requirements according to organization standards.

  • Tracks client attendance at medical appointments and patient navigation sessions and initiates outreach and missed appointment procedures, as necessary.

  • Communicates with and travel to schools, emergency departments, medical practices, pharmacies, and participant homes in the Rochester area.

MINIMUM EDUCATION & EXPERIENC E

  • Bachelor's degree in healthcare related field required

  • Or equivalent combination of education and experience

  • 1 year experience in community health, human services, and/or patient liaison/advocate roles preferred

KNOWLEDGE, SKILLS AND ABILITIES

  • Communication required

LICENSES AND CERTIFICATIONS

  • BCPA-Board Certified Patient Advocate upon hire preferred and

  • DL NUMBER - Driver License, Valid and in State plus a satisfactory driving record and access to reliable transportation upon hire required
