HCR Home Care Certified Home Health Aide - Monroe County Future Opportunities in Rochester, New York
Role and Responsibilities
Provide personal care and supportive services to individuals in the community , in the privacy of their home.
Essential Functions
Provide health - related services as outlined in the plan of care , authorized by a physician , and established by a registered nurse or licensed therapist , under the direction of the Aide Matrix established by the NYS Department of Health.
Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.
Immediately report changes in client’s condition to supervising professional.
Ensure the safety of the client , clie nt’s family and/or the employee.
Maintain HIP A A.
Complete and submit necessary patient documentation including charting tasks and observations on android device.
Attend ongoing in-service programs in a timely manner , to maintain certification.
All other duties as assigned.

Education Requirements
High School diploma or GED preferred .
Completion of an approved program of PCA or HHA certification as designated by NYS Department of Health
Qualifications and Requirements
Valid Driver's License preferred; reliable transportation required.
Must have proof of completion (Original HHA Certificate) of an NYS Dept of Health accredited training program .
Must have worked in a certified HHA capacity for a LHCSA within the last 24 months (HCR will verify via the NYS DOH's Home Health Aide Registry) .
Work Environment
The HHA is primarily in a non-office setting and may be exposed to outdoor conditions.
The working conditions are classified as medium work:
Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
