Nuvance Health Geriatric Nurse Navigator in Poughkeepsie, New York
Description
Summary:
The Geriatric Nurse Navigator will ensure seamless care transitions, improve outcomes, and support the Emergency Department team in meeting Geriatric Emergency Department AccreditationGEDA standards on delivering age-appropriate, high-quality care.
Responsibilities:
1) Act as the primary advocate and resource for geriatric patients and their caregivers during the ED visit. This includes age-friendly care practices and improvements in the ED environment to enhance safety and comfort for older adults.
2) Conducts assessments for key geriatric syndromes, including cognitive impairment, delirium, fall risk, polypharmacy, and functional status.
3) Collaborate with multidisciplinary teams to create individualized care plans ba6)sed on geriatric-specific needs and evidence-based practices. Also, develops and implements safe, effective discharge plans in collaboration with social workers, case managers, and community resources.
4) Provide patient and family education on diagnoses, treatment options, discharge plans, and follow-up care. Post discharge follow up with high-risk older adults to address any care gaps.
5) Coordinate with primary care providers, geriatric specialists, and post-acute care facilities to optimize care transitions and prevent readmissions. Build partnerships with community organizations to promote access to geriatric resources and services.
6) Drive quality improvement initiatives to achieve or maintain GEDA accreditation (Level 1, 2, or 3).
7) Ensure compliance with GEDA standards, including geriatric-focused staff education, clinical protocols, and environmental modifications in the ED.
8) Collect, analyze, and report data on geriatric-specific metrics, such as delirium screening, fall prevention, and patient outcomes.
9) Act as a champion for geriatric care initiatives and provide feedback to leadership on opportunities for improvement. Provide training and education to ED staff on best practices for geriatric care, including the recognition and management of geriatric syndromes.
