TO APPLY CALL (360) 207-0966 PART-TIME HEALTH HOMES CARE COORDINATOR wanted for Olympic Area Agency on Aging (O3A) based in Port Townsend, WA. 25hrs./wk. Non-Exempt. Salary range is $57,463 -- 75,396 annually. Full benefits package and WA PERS retirement plan. Provides support for designated health home clients. Care coordination responsibilities include assessment, care planning and monitoring of client status, and implementation and coordination of services. Facilitates interdisciplinary consultation, collaboration and care continuity across care settings. Some travel required in Jefferson and Clallam Counties. Master's Degree in behavioral or health science and one year paid on the job social service experience, OR Bachelor's Degree in behavioral or health sciences and two years of paid on the job social service experience, OR Licensed Practical nurse licensed to practice practical nursing under chapter 18.79 RCW with at least two years of related job experience, OR Bachelor's Degree and four years of paid on the job social service experience. Training in Coleman CTI or other coaching modality is preferred. Requires current WDL & insured vehicle. Open until filled, candidates encouraged to apply promptly.
