Honor Community Health Recovery Coach in Pontiac, Michigan

Honor Community Health is a 501c3 Federally Qualified Health Center co-located within 20 locations. Our mission is to provide for the health and wellness needs of the underserved of Oakland County through the provision of comprehensive, integrated primary, behavioral health, and dental care. Position Description

Under the supervision of the Project Director for the CCBHC the CCBHC Recovery Coach will work as a team member in a primary care setting that serves all members in the community through support and coaching. This position will provide and maintain a supportive, hopeful, empathic, and engaging environment for individuals seeking help with substance use issues. As a member of a multidisciplinary team, the Recovery Coach will provide recovery coaching to people that results in engaging them in ongoing supportive services in and communicating the message of hope and the possibility of recovery and healing.

Essential Functions

  • Use recovery coaching and recovery pathways to maximize the opportunity to create recovery partnerships and recovery environment, engages and motivates people serve in the recovery process, link people serve to appropriate aftercare services and community resources utilizes the principles of hope, choice, empowerment, recovery environment, meaning and purpose. provides strength-based trauma inform intervention.

  • Working closely with the interdisciplinary team completes all documents in the electronic health record promotes a team environment utilizes person center planning contributes to a welcoming atmosphere for the people we serve.

  • Participate in staff meetings, in-service training, and supervision.

  • Work cooperatively with members of the team, adjunct providers, and other agencies to ensure timely and accurate service provisions are performed without regard to the above listed items.

What are we looking for?

  • High School Diploma required.

  • Currently Certified by the State of Michigan as a Peer Support Specialist preferred.

  • Bilingual (Spanish/English) preferred.

  • Experience in human services, social work, and/or public health preferred.

  • A minimum of one year of experience working in a clinical environment requires or equivalent experience.

  • Experience working for a community health center or Federally Qualified Heath Center preferred.
