Clinical Pharmacist Specialist, Infectious Diseases.
Education or Equivalent Experience:
· Bachelor of Science in Pharmacy OR Doctor of Pharmacy from an accredited College of Pharmacy (required)
· PGY-1 Pharmacy Practice Residency (required)
· PGY-2 Pharmacy Practice Residency Infectious Disease (required)
· PA Pharmacist License (required)
Responsibilities:
· Provide a full range of pharmaceutical care services to patients managed by the Infectious Diseases Consult Services and Antimicrobial Stewardship Program.
· Participate in ID consult rounds, evaluate, monitor, and modify antimicrobial therapy to optimize cost-effective patient outcomes.
· Serve as a subject matter expert for medical personnel, the Pharmacy Department and the Infectious Diseases department.
· Coordinate pharmacotherapy activities and education opportunities for department pharmacists and other healthcare professionals.
· Initiate or participate in quality improvement projects and recommend updates/modifications as necessary to optimize pharmacy services.
· Precept different types of learners, including pharmacy students, PGY—1, and PGY—2 (ID, CC, ED) residents.
