About the Role

The role of the Medical Social Worker entails delivering behavioral health care management services within an ambulatory primary care practice, catering to diverse patient populations across all age groups. Responsibilities include coordinating care to ensure patients and their families have access to community services, administering behavioral health screenings, and arranging more detailed assessments as needed. Integrating evidence-based guidelines into the development of individualized, patient-centered care plans, monitoring progress to optimize health status, and providing brief interventions for necessary lifestyle and behavioral changes. Responding to crisis situations, identifying high-risk psychosocial needs, and collaborating with patients, families, and interdisciplinary care teams to understand the impact of illness on mental and emotional health. Offering brief, solution-focused prevention, treatment, and recovery services for patients with mild to moderate risk factors, while ensuring the delivery of quality, efficient, and cost-effective team-based health care services. Providing culturally competent clinical services relevant to patients and families, with duties varying depending on the area of assignment.

 

Qualifications:

  • Master of Social Work degree preferred, with one-year recent medical or mental health experience, or equivalent Master's degree.
  • Licensed Master of Social Work (LMSW) or Limited Licensed Master of Social Work (LLMSW) in the state of Michigan.
  • Proficiency in word processing, spreadsheets, presentation programs, and other relevant software.
  • Demonstrated customer-focused interpersonal skills for effective interaction with practitioners, interdisciplinary health care teams, community agencies, patients, and families with diverse opinions, values, and cultural ideals.

     

Responsibilities:

Function autonomously on behalf of the providers under the Bronson system, collaborating with providers for all identified responsibilities.

Identify appropriate patients for intervention.

Assess patient needs and coordinate services and referrals.

Evaluate crisis situations and provide appropriate interventions based on immediate social/emotional needs.

Perform screenings based on patient population and need.

Collaborate with the patient care team to recommend and provide immediate treatment and solution-focused interventions.

Develop comprehensive individualized care plans with targeted interventions.

Conduct regular follow-ups with patients to evaluate progress and response to the plan of care.

Provide patient self-management support to empower patients and families for self-care.

Educate patients of all ages based on knowledge of growth/development across the lifespan.

Promote healthy behaviors and provide navigation assistance with community resources.

Link patients to social services programs and make referrals to resolve emotional, social, or financial problems impacting health care status.

Implement clinical interventions and protocols based on risk stratification and evidence-based guidelines.

Thoroughly document each case into the patient's medical record.

Identify and provide for learning needs of patients, families, and healthcare coworkers.

Work with ambulatory care management to evaluate processes and propose improvement strategies to enhance patient care.
