TAY Peer Partner

Our mission at Sevita is to create a world where every person has the right to live well. As a TAY Peer Partner, you'll provide guidance to transitional aged youth (TAY) being served through the system by utilizing personal prior experience to help foster a relationship of trust and support, identify strengths and unmet needs, to build connection, confidence and hope for the youth.

$38,022 Annually (Salary plus overtime)

Responsibilities:

  • Advocacy and liaison services between the TAY youth, clinician, treatment staff, community agencies, services, families and child(ren)

  • Provide services in the home and community

  • Ensure families are properly supported through the system of care, probation or mental health services and connect families to resources/services in the community

  • Participates as a member of the child and family team and assists the TAY youth in bringing up and discussing difficult issues at meetings

  • Leads or co-leads youth groups

  • Attends and participates in trainings, treatment planning meetings, court review hearings, team decision-making, IEP and other meetings

  • Documentation and record keeping

  • Performs additional duties and activities as required

    Qualifications:

  • High school diploma or equivalent required

  • Prior personal experience as a youth who has been served through the system of care, probation, or mental health services required

  • Experience in a mental health setting preferred

  • Reliable transportation, auto insurance, registration, and a valid driver’s license

  • Successful clearance of local and/or state background checks

  • COVID-19 full vaccination required
