ProMedica Health System Inpatient Pharmacist - Bay Park Hospital - Per Diem - Variable in Oregon, Ohio

ProMedica Bay Park Hospital caters to communities east of the Maumee River. Our campus includes a state-of-the-art community hospital with 91 private beds and a professional office building dedicated to doctors' offices and outpatient rehabilitation, as well as a treatment and diagnostic center. The hospital itself was designed with patient comfort and healing in mind, with wider hallways, single-patient rooms and an abundance of natural light.

POSITION SUMMARY

Provides pharmacy services, to include: inpatient drug distribution, IV admixture, drug information, pharmacokinetics, drug therapy monitoring, patient education, and controlled substance regulation. Responsible for providing direct supervision of supportive personnel within the Department of Pharmacy. Actively participates in the creation of an environment that fosters patient, family, physician, and staff satisfaction. Ensures decisions made are based on the Mission and Values of ProMedica.

ACCOUNTABILITIES

  1. Interprets and processes all medication orders. Prepares and dispenses medications, including sterile, chemotherapy, and parenteral nutrition preparations safely and accurately using appropriate techniques.

  2. Sustains the formulary by minimizing nonformulary procurements, utilizing the therapeutic substitution policy and promoting rational medication therapy selection. 3. Monitors all new medication orders against the patient s medication profile for contraindications, drug drug interactions, drug food interactions, drug laboratory test interactions, allergies, and appropriateness of drug, dose route, frequency, and therapeutic duplications. Resolves identified problems with the physician or nurse; and documents these activities as appropriate.

  3. Answers all drug information questions accurately using drug information resources available in the department and documents these activities as appropriate.

  4. Reads, extracts, and interprets information in patient charts accurately.

  5. Provides professional review, quality control checks, and/or direct provision of service for: all unit dose packaging program, all admixtures and sterile products including formulation, stability and sterility consideration, extemporaneous compounds, medication order entry, floor stock orders and controlled substances.

  6. Documents all clinical activities and interventions accurately and completely.

  7. Obtains required continuing education for state licensure.

  8. Provides clinical consultation and clarification to practitioners; recommends evidence-based medication therapy regimens and monitoring plans; and suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed. Performs clinical services as required including pharmacokinetic dosing, renal dosing, etc.

  9. Detects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner.

  10. Conducts patient education/counseling to inpatients and/or outpatients as requested.

  11. Collaborates with other team members in meeting departmental goals. Establishes and maintains mechanisms for communication and constructive problem solving with staff. Utilizes a team approach in decision-making and problem solving. Builds trust and mutual respect.

  12. Keeps director informed of any unexpected developments that may affect patient care or the budget of the department.

  13. Supervises and directs pharmacy support personnel; verifies the daily activities of pharmacy technicians; and participates in the performance management process as requested. Supervises and instructs students.

  14. Adheres to state and federal laws.

  15. Performs all duties and responsibilities in an ethical and responsible manner.

  16. Supports the department in providing a vision for its direction and embraces the change process to achieve the vision.

  17. Effectively makes decisions to achieve desired outcomes.

  18. Coaches and facilitates the growth and development of employees within the department.

  19. Directs daily operations to achieve goals for the department.

  20. Strives for continuous improvement by focusing on key department systems.

Education: Bachelors Degree in Pharmacy

License: Licensed within the state(s) of practice

PREFERRED QUALIFICATIONS

Education: Pharm.D. degree and residency training

Skills: Prefer experience in hospital pharmacy services including unit dose, IV admixtures, and pharmacokinetic dosing.

WORKING CONDITIONS

Physical Demands: Must be able to lift light materials. Non-material handling of continuous sitting and reaching; frequent standing; occasional squatting, kneeling, walking, and stairs. Must be able to move about and between workstations.
