Position Summary

Guide the child and family team in planning, facilitating, and coordinating the individualized Service Coordination Plan by developing an ongoing, supportive relationship with the family and facilitating communication and coordination between team members.

Essential Functions/Major Responsibilities

Assist in the development and implementation of the child and family team, this includes:

  • Maintaining regular contact with the child, family, service providers, and representatives of other systems in which the child is involved.

  • Provide case management services such as assessing needs, linking to services, monitoring, and coordinating services from multiple providers and facilitating crisis response.

  • Maintaining the child’s Integrated Service Array case file and is responsible for gathering and transmitting fidelity measures and utilization, family satisfaction and outcome data.

  • Collaborate with the child and family team in developing and implementing a transition plan from the Integrated Service Array.

Provide linkages and referrals to families, including:

  • Create linkages to and monitors transitions between levels of care.

  • Provide support and consultation to families about system involvement and advocacy.

    Other functions:

  • Interact and convene with community partners and resources as necessary to assure population needs are being met.

  • Lead, convene, and participate in team meetings, trainings, and activities to further the health of the community.

Typical Qualifications

Minimum Education Level

Bachelors

Specifics: in social behavioral field with special training in behavioral health.

Minimum Experience Level

Two- Three years

Specifics: of clinical treatment experience in the pediatric and/or adolescent behavioral health field.

Certifications

Certification(s) required

Specifics: Classified as a Qualified Mental Health Associate.

Driver’s License

Oregon Driver's License required

Specifics: Must obtain within 30 days of hire.

Desirable Qualifications: Bilingual and/or bicultural competencies for Latino population.

Knowledge of:

  • Principles and practices of wraparound delivery systems and case management.

  • Understanding diagnosis and treatment of children with problems affecting their environment along with developmental stages.

  • Principles and practices of behavioral health treatment.

  • Rules and regulations guiding community behavioral health practices.

Skill or ability to:

  • Exercise sound judgment in appraising situations and decision making when developing Intensive Service Array coordination plans.

  • Convey information, ideas, and facts both orally and in writing to supervisors, colleagues, and individuals, inside and outside the county, using language and a format the audience will best understand.

  • Utilize computers and other technology to communicate in written and verbal forms (e.g., word processing, texts, emails, spreadsheets, department-specific software).

  • Effectively relate well with others, including supervisors, colleagues, and individuals inside and outside the county. Exhibit a professional manner in dealing with others and working to maintain constructive working relationships.

  • Take personal responsibility for the quality and timeliness of work. Show up to work on time, and follows instructions, policies, and procedures. Meet productivity standards, deadlines, and work schedules.

Job Conditions and Physical Demands

WORKING CONDITIONS

Job Conditions: Normal office setting with the possibility of hours in the evening or on weekends. Driving conditions may vary due to extremes in weather. May have some exposure from bodily fluid/lab samples. May be encountering unstable/hostile clients in their homes, community settings, or clinics.

Travel: Tasks require travel in the community.

  • Physical Ability: Tasks involve the ability to exert very moderate physical effort in light work, typically involving some combination of stooping, kneeling, crouching and crawling, and which may involve some lifting, carrying, pushing and/or pulling of objects and materials of moderate weight (12-20 pounds).

Lincoln County offers a comprehensive benefit package including vacation, holiday, and sick leave as well as medical, dental, vision which is paid at 90% by the County and 10% by the employee. Other fully paid benefits include life, AD&D, long-term disability insurance. Other optional insurances are available. A 401K retirement plan is also provided. The County's current contribution rate is 11%.

Regular and Trial Service employees working between 20 - 39 hours per week on a regular basis are eligible for pro-rated employee benefits.
