Project FIND Social Work Case Manager Supervisor in New York, New York

Please visit our website at: www.projectfind.org.Job Summary:Case Management services help older persons with functional impairments gain access to appropriate services, benefits and entitlements needed to age safely at home and to maintain their quality of life. The Case Management Supervisor (Bilingual preferred), an MSW will have responsibility for supervising 5 case managers and will ensure that all case management clients receive a timely, thorough, strengths-based assessment; and that care plans are developed that reflect culturally sensitive options that make efficient use of community resources. She/he will be responsible for creating a training and development plan for each case manager on his/her team. Included in this plan will be assurance of completion of all NYC Aging training requirements and continued support of ongoing staff development.The Case Management Supervisor will report to the Director of Community Services and will develop and maintain external partnerships with other organizations to expand resources and strengthen supports.General Responsibilities:-Provide case managers with on-going supervision.-Approve and sign service plans.-Complete supervisory case record review as required by NYC Aging or as needed.-Ensure the case managers' records maintain up to date information.-Ensure case managers obtain the required NYC Aging training hours.-Individual supervision once a month or as needed.-Provide new case managers with NYC Aging Case Management policies and Project FIND’s protocols and standards.-Provide on-going support to case managers when dealing with a difficult client.-Accompany case managers on home visits when deemed necessary.-Advocate and intervene with other agencies on behalf of the clients.-Complete NYC Aging training required hours as well as agency-wide requirements.-Attend all required staff and/or agency meetings.-This position is not limited to the list above as the supervisor may be required to perform other appropriate tasks as needed.Qualifications:-LMSW (preferred) or related master’s level degree (e.g. social services, public administration, nursing or public health) and 2 years supervisory experience or-Bachelor’s level degree and at least 4 years of experience (2 of which must be supervisory) working in the field of aging.-Must demonstrate cultural competence, be knowledgeable and sensitive to the diverse needs of the communities served, including clients with visual and/or hearing impairments.-Must be able to multi-task.-Bilingual English/Spanish or English/Chinese speaker preferred and must demonstrate verbal and written competencies.-Ability to gather, summarize and review data for reports, find solutions to challenges related to the clients.-Ability to apply principles of logical thinking to a wide range of intellectual and practical problems.-Must be able to prioritize work.-Demonstrate excellent interpersonal and communication and strong customer-service skills.-Must exercise excellent follow-through on assignments.Schedule:Monday - Friday from 9:00 a.m. to 5:00 p.m.Occasional evening, holiday, and weekend work.

Minimum Salary: 72,000 Maximum Salary: 75,000 Salary Unit: Yearly
