The NYC Department of Health is expanding access to home visiting services to improve maternal and child health, addressing racial and socioeconomic disparities. The program focuses on prenatal to postpartum care and reducing maternal morbidity and mortality. We seek a leader who can inspire and empower others to achieve these goals.

DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:

  • Ensure all client referrals are promptly and appropriately assigned to home visiting staff.

  • Contact clients to conduct initial intakes, gather essential information, and schedule home visiting appointments.

  • Identify and connect clients to community resources that support women and families residing in shelters, addressing their unique needs.

  • Conduct home visits to perform follow-up assessments and surveys, distribute cribs, and provide other program materials to families, particularly those who have recently given birth

  • Offer screenings and health education on maternal and infant health topics, including breastfeeding, safe sleep practices, early childhood development, and environmental assessments.

  • Follow up with referral partners to obtain updates and outcomes for client referrals, ensuring comprehensive service coordination.

  • Collect, enter, and maintain accurate program data; monitor, review, and manage case assignments to ensure timely service delivery.

  • Actively participate in program-sponsored events, such as crib distribution initiatives, health fairs, and stakeholder training sessions.

  • Provide support to senior management by assisting with special projects, initiatives, and additional assignments as needed.

PREFERRED SKILLS:

  • Demonstrated expertise in community outreach, early childhood development, breastfeeding education, and/or maternal-child health education.

  • Proficiency in data collection and strong familiarity with Microsoft Office applications (e.g., Word, Excel, Outlook).

  • Exceptional written and oral communication skills, with the ability to engage effectively with diverse populations.

  • License Requirements: A valid New York State motor vehicle driver's license is required for certain assignments. This license must be maintained throughout the duration of employment if applicable.

  • Fluency in English is required; proficiency in Spanish and/or French is strongly preferred.

PUBLIC HEALTH ADVISER - 51191

Qualifications

  1. A baccalaureate degree from an accredited college, including or supplemented by twelve semester credits in health education, or in health, social or biological sciences; or

  2. A baccalaureate degree from an accredited college, and six months of full-time satisfactory experience in a health promotion or disease intervention/prevention program, performing one or more of the following: interviewing, conducting field investigations, assessing health risks, making referrals, or collecting and analyzing epidemiological data; or

  3. A four-year high school diploma or its educational equivalent, and four years of full-time satisfactory experience as described in "2" above; or

  4. Education and/or experience equivalent to "1", "2" or "3" above. Undergraduate college credit can be substituted for experience on the basis of 30 semester credits from an accredited college for one year of full-time experience. However, all candidates must have a four-year high school diploma or its educational equivalent, and either twelve semester credits as described in "1" above or six months of experience as described in "2" above.

Additional Requirements

A. To be assigned to Assignment Level II, candidates must have, in addition to meeting the minimum qualification requirements listed above, at least one year of experience as a Public Health Adviser, Assignment Level I, or at least one additional year of experience as described in Qualification Requirement "2" above.
