Early Intervention & Pre-School Evaluation Coordinator

  • New York, NY

  • The Evaluation Coordinator is responsible for assisting with the Early Intervention and the Committee for Preschool Special Education evaluation process beginning at the time of the assignment up through the initial IFSP meeting or CPSE meeting with a district administrator.

Essential Functions

  • Perform the intake

  • Assign the needed assessments to AAK Evaluators to perform

  • Enter the child and therapist information into the EIP tracking sheet to manage due dates.

  • Request and obtain medical exam record for each child evaluated.

  • Verify referring doctors on Medicaid Excluded list.

  • Ensure that scripts are obtained and completed for Speech, OT, PT and Psychological evaluations as needed

  • Give completed Evaluation packets to QA staff for review.

  • Enter data into the CLAIMS system for billing and track for device authorizations for payment of evaluation services.

  • Obtaining all relevant child information from parent or Service Coordinator in order to begin the evaluation process

  • Obtaining the C1-P (consent form) filled out completely by parent and signed as well as the Medical form

  • Assigning Psychological Evaluator based on home language child is exposed to (Monolingual/Bilingual) and assigning additional evaluations based on the Psychologist request based on child’s developmental needs/concerns

  • Gathering all additional evaluations with EIP goals to submit to Psychologist for SUMMARY, IEP, and OUTCOMES forms to be created.

  • Obtaining OT/PT Rx if a Gross Motor evaluation was conducted and Speech referral forms if SLP evaluation was conducted

Competencies

  • Microsoft Office including Word, Excel, Outlook, and Teams.

  • Ability to multi-task and work efficiently in a fast paced environment

  • Strong teamwork and interpersonal skills

  • Fluent in both English/Spanish

Required Education and Experience

  • High School Diploma, GED or equivalent

Preferred Education and Experience

  • Bachelor’s Degree in a Human Service related field

  • 2+ years’ experience in Early Intervention and/or CPSE programs
