Location: Mt. Vernon, NY

Hours: 35- Mon-Fri 9am-5pm

Pay Rate: $20/hr

Position Summary: Work primarily in various community settings including the homes of program participants who are adults in mental health or substance use recovery. Provide advocacy, outreach, recovery and crisis support. Assist participants to regain independence within the community and mastery over their own recovery process. Assist participants to identify their own individual warning signs of relapse and identifying and developing individual coping strategies and crisis prevention tools. Provide supportive peer counseling with respect to mental health, substance abuse, rehabilitation strategies and self-help opportunities. Use tools such as the Wellness Recovery Action Plan (WRAP) to assist peer s in creating their own individual wellness and recovery plans.

Responsibilities:

Support a high-performing culture aligned with TGCW core values and B.A.S.I.C Tenets: Balance, Accountability, Satisfaction, Inclusion, and Communication.

Complete assessments, service plans, daily work logs and case notes.

Meet regularly with participants in their homes, communities and office providing peer service and psychosocial rehabilitation services.

Provide counseling services including teaching coping skills, relapse prevention strategies, wellness management and social skills to help participants become more independent and achieve their goals.

Teach independent living skills.

Assist participants to secure housing.

Connect participants to community resources.

Work with participants’ natural support system.

Communicate/collaborate with participants’ treatment providers.

Participate in staff meetings, case conferences, and relevant trainings.

Other duties as assigned.

Qualifications: Certification or Provisional Certification as Peer Specialist or Recovery Peer Advocate . Adequate writing and computer skills for completion of reports, case notes, and other documentation as needed in electronic health reporting system. Able to navigate telehealth via video and conference calling. Personal lived experience with mental illness or substance abuse. Willingness to share personal experience with others. Knowledge of travel within Westchester County,NYS Driver’s Licenseand ability to drive agency vehicles preferred.

Travel Requirements: Generally, 60-70% in the field.
