Job ID359841 LocationTwin Cities Job FamilyHealth Care Full/Part TimeFull-Time Regular/TemporaryRegular Job Code8301CH Employee ClassCivil Service About the Job Community Health Worker - Harm Reduction Outreach Specialist About the Job The Community Health Worker will support the Mobile Health Initiative (MHI) by working closely with the Community Engagement Nurse, performing street outreach to members of our community who are impacted by substance use disorder and may be experiencing homelessness. You will play a vital role in assisting the community engagement nurse to promote the health, safety, and dignity of the communities we serve while working with partner community organizations that perform harm reduction street outreach. This position works with vulnerable patient populations that have traditionally been marginalized and stigmatized, building trust and meeting their needs in the moment while performing outreach. MHI works with clients in the Twin Cities and northern Minnesota. This position is a hybrid 100% time position. Four days per week will be in-person and one day per week will be remote to plan events and coordinate with MHI staff around supply purchasing and mobile unit cleaning/maintenance. Most of the work is in the Twin Cities metro area and may be in Northern Minnesota depending on needs and the applicant's desire to travel. Transportation to and from events is required and driving the mobile unit is expected. Travel will be reimbursed if outside of the metro area. Responsibilities Client Liaison (35%) Conducts outreach efforts with nurse and community partners to engage with individuals who use drugs, building trusting relationships and providing resources for social determinants of health as desired. Educating clients on safer use of drug use practices (harm reduction), overdose prevention, healthcare navigation, and connection to available resources for pharmacies, substance use disorder (SUD), detoxes, and treatment centers. Assisting nurse in identifying and assisting patients who want to initiate SUD treatment, providing referrals, and coordinating care between MHI and community clinics Provide patients rides to SUD appointments as needed and per a TBD safety protocol and up to the discretion of the CHW. Answers and returns calls to clients requesting assistance from MHI staff. Street Outreach & Coordination (50%) Building and/or maintaining partnerships with local community organizations and other agencies learning their service goals and missions. Documenting patient interactions in RedCAP, an application used to maintain accurate service records. RedCAP training will be provided. Assist with data entry such as number of services offered, follow-up care given, supply inventory, and ordering of supplies as needed. Assist with loading/unloading of the mobile unit. Partner with the nurse to determine the needed education materials and supplies, re-stocking the mobile clinic. Driving the mobile unit, after appropriate training, to events as needed. Participate in the shared responsibility of mobile unit cleaning and maintenance. Represent UMN and MHI to community partners and members, communicating our mission and providing service descriptions and explanations. Assist community partners and clients with event logistics, helping with event flow and troubleshooting any issues. Administrative (15%) Participating in training and professional development opportunities to improve outreach and harm reduction skills. Provide daily operational support by attending and scheduling routine meetings. Answering the team's cell phone and email, as needed. Ensure feedback on community needs is communicated to appropriate channels. Qualifications Required Qualifications: BA/BS or a combination of related education and work experience to equal at least four years, Years of experience to include two years of health-related experience and 1 year of experience working with diverse, unsheltered or immigrant communities omputer proficiency, including Microsoft Work, Excel and PowerPoint Valid driver's license Excellent communication and interpersonal skills along with the ability to connect with individuals from diverse backgrounds that are often stigmatized. Must be able to lift 25 lbs for supply transportation Preferred Qualifications: Interest in community and public health along with providing health education Ability to practice non-judgment while building rapport with marginalized individuals. Strong passion and commitment for harm reduction, social justice, decreasing health equity gaps and serving vulnerable communities. Proficiency in Somali, Oromo, Spanish, Hmong or another language Peer recovery support specialist certification or training Self-motivated, goal-oriented, and mindful of deadlines Administrative or customer service experience Prior experience working in a complex environment supporting multiple individuals Community Health Worker Certification or prior professional experience. Experience driving large vans or trucks (CDL not required, but training will be provided) About the Department Mission Establish mobile health partnerships between community organizations and the University of Minnesota by offering support to increase access to healthcare services and improve health outcomes for Indigenous, refugee, immigrant, and migrant communities and those underserved and disproportionately impacted by social injustice and COVID-19.
