IMPACT Senior Housing Facilitator in MILWAUKEE, Wisconsin

JOB REQUIREMENTS: POSITION SUMMARY: Provide quality professional services to internal and external customers with IMPACT Coordinated Entry. The Senior Housing Facilitator creates and maintains relationships with emergency shelters, Safe Havens, and warming sites and groups that are likely to refer to shelter, like street outreach groups, health systems, and case managers, and Rapid Rehousing and Permanent Supportive Housing programs. This position manages the housing and emergency housing call, Monday-Friday and facilitates group consensus for all housing placements within the Coordinated Entry System. The Senior Housing Facilitator acts as a liaison between the CES and other social service professionals in the Milwaukee County community seeking shelter for mutual clients. RESPONSIBILITIES: Prepare and facilitate a conference call every weekday morning with CoC partners. Complete day to day bed and reporting needs for Coordinated Entry. Contact all appropriate agencies with bed count via phone, email, or text. Showing competence in the shelter priorities for placement, work toward consensus during the conference call for housing and emergency housing placement. Update the Coordinated Entry team and all community partners at end of shift: Who was placed? Who still needs to be placed and in what priority. Any other information that is necessary to serve our partners and clients. Prepare and facilitate Safe Haven placement staffings, as needed by the CE Team. Facilitate priority conversations using the Safe Haven prioritization list or other reports. Move the group to consensus during the call for Safe Haven placement. Work with the CE Data Specialist to ensure accurate data is complete in HMIS and used to ensure accurate referrals to Safe Haven providers. Prepare and facilitate housing placement staffings, as needed by the CE Team. Facilitate priority conversations using the housing prioritization list or other reports. Move the group to consensus during the call for housing placement. Work with the CE Data Specialist to ensure accurate data is complete in HMIS and used to ensure accurate referrals to housing providers. Coordinate referrals from the CE team, street outreach workers, community members, and self-referrals to emergency shelters and other emergency placement options based on vulnerability and need. With the assistance of the Coordinated Entry Manager, create and maintain solid relationships with CoC and community partners for the purposes of smooth, effective, and prioritized placement into available emergency and permanent housing. Specifically, work with the CE Health Systems Liaison regarding Housing is Health referrals, assessments, and placements, including healthcare and other related external relationships. Participate in relevant community workgroups. Obtain and maintain accurate bed counts for CoC Partners. Participate in trainings and meeting related to homelessness, HMIS and any other training or meetings with the placement of people and families into appropriate CoC agencies. Assure that data entered into the Wisconsin HMIS database is accurate, up-to-date and entered according to the standards and practices required by the Milwaukee County Continuum of Care and all governing bodies as well as tracking and reporting as assigned by Coordinated Entry Manager. Meet all funding requirements to include trainings and other annual requirements. Standard IMPACT Requirements: Responsible for protecting client confidentiality and adhering to all state and federal laws and regulations regarding the protection to AODA and mental health information. Protects against unauthorized access, modifications, destruction, and disclosure as defined by IMPACT and relevant federal, state, and local laws. Comfortable with public speaking. Flexibility in approach; ability to broadly apply concepts to new situations. Excellent problem solving skills. Exceptional verbal and written communication skills. QUALIFICATIONS: Bachelor\'s degree in social work or human service studies preferred but a strong background of applicable work experience may substitute Experience working with people experiencing homelessness strongly preferred Computer software expertise: Word, Excel; HMIS (Clarity) experience a plus; Ability to learn new software systems Reside in the State of Wisconsin and within 50 miles of IMPACT Training: This is a full time, hybrid, non-exempt position within IMPACT. General work hours will be Monday through Friday, between the hours of 8AM to 5PM.
