JOB REQUIREMENTS: Position Description: The Medical College of Wisconsin\'s Department of Medicine, Division of Nephrology is seeking a Nurse Practitioner or Physician Assistant to provide primary or specialty medical care within defined scope of practice under the supervision of a licensed physician and in accordance with MCW/FMLH/CHW policies and procedures. Located in Milwaukee, WI at Froedtert Hospital and Outpatient Dialysis Centers. The Advanced Practice Provider (APP) works as part of a multidisciplinary health care team, providing a broad range of diagnostic and therapeutic services for the patients of the inpatient services as a member of the Department of Medicine, Division of Nephrology. The APP will have a strong foundation in evidence-based practice and ensure an environment of safe quality care. In this role you will be responsible for the following: Collaborates with interdisciplinary health care teams in diagnosis and treatment of conditions requiring medical intervention in inpatient environment as a member of the General Nephrology service. Writes accurate and timely progress notes, procedure notes, and/or admission and transfer orders. Order, perform, and interpret screening and diagnostic procedures and tests, and appropriately communicates and documents information. Privileged to perform procedures beyond core as identified by the Nephrology services. Facilitates and communicates with patient, family and staff to promote continuity of care across the continuum of care. Attend required training and meets key regulatory requirements, including those of the privileging institution, CMS, and the Join Commission. Evaluates quality outcomes for the patients receiving care of this team through participation in the development of quality indicators and outcomes dashboard. Acquires information on the most current development and trends in the Physician Assistant field and/or Nurse Practitioner field, through educational programs, in-services, professional associates, and current literature Core Advanced Practice Responsibilities: Perform comprehensive histories and physical exams, identifies normal and abnormal findings, including physical, cognitive, and psychosocial status. Order, perform, and interpret screening and diagnostic procedures and tests, and appropriately communicates and documents information Write accurate and timely progress notes, procedure notes, and/or admission and transfer orders Develop pharmaceutical and non-pharmaceutical treatment plans.
