Our team is passionate about providing comprehensive and compassionate care to meet the unique needs of each senior we serve. We are currently seeking a skilled Geriatric Care Manager to join our team. The ideal candidate will have a strong background in geriatric care, excellent communication skills, and a passion for making a difference in the lives of the elderly.   Job Description: As a Geriatric Care Manager, you will play a critical role in coordinating and managing the care of elderly clients. You will assess their needs, plan and implement care strategies, coordinate with healthcare providers, and offer support and guidance to families. Your goal will be to enhance the quality of life for the elderly and ensure they receive the highest standard of care.   Responsibilities: - Conduct comprehensive assessments of clients' health, functional status, and psychosocial status. - Develop, implement, and monitor personalized care plans in collaboration with clients, families, and healthcare providers. - Serve as a liaison between clients, families, and healthcare providers, facilitating communication and ensuring continuity of care. - Advocate for clients' needs and preferences with healthcare providers and other stakeholders. - Provide guidance and support to families, helping them navigate healthcare systems and make informed decisions about care. - Monitor clients' conditions and adjust care plans as necessary. - Ensure compliance with all relevant laws, regulations, and ethical standards related to geriatric care. - Maintain accurate and up-to-date client records.   Requirements: - Bachelor's or Master's degree in Social Work, Nursing, Gerontology, or related field. - License or certification in a relevant discipline (e.g., LCSW, RN, LPN) preferred. - Minimum of 2 years of experience in geriatric care or case management. - Strong knowledge of the healthcare system and resources available for the elderly. - Excellent communication and interpersonal skills, with the ability to work effectively with diverse clients and families. - Strong organizational skills and the ability to manage multiple cases simultaneously. - Compassionate and patient, with a genuine interest in improving the lives of the elderly. This position is part time, covers our Michiana service area and is hybrid home-based with visits to clients, as needed.
