Job Overview
The Birthplace RN Care Coordinator provides comprehensive care coordination of those patients that are anticipating discharge from the Birthplace. The Birthplace RN Care Coordinator assesses the patient’s plan of care and develops, implements, and documents the necessity of resources, facilitating options and services to meet the patient and their infant’s health care needs post discharge. Coordinates care efforts for a safe, effective, efficient, and patient centered transition along the health continuum. The intensity of care coordination provided is situational and appropriate based on patient need and payer requirements.
This position has responsibility to determine how to best accomplish functions within established procedures, consulting with leader on any unusual situations. Internal customers include all levels of nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External customers include physicians and their office staff, payers, community agencies, provider networks, and regulatory agencies.
This is a 1.0 FTE, working the Day shift, with no weekend requirement.
Job Expectations:
Identifies needs and implements quality standards of nursing care for the obstetric and neonatal patient.
Accesses, analyzes, plans, and coordinating follow-up care.
Facilitates timely referrals and fosters positive relations with community providers
Collaborates with appropriate staff to provide coordinated transition of care across the healthcare continuum.
Collaborates with health care team partners and patients/family to manage the patient discharge plan.
Facilitates timely referrals and fosters positive relations with community providers
Identify and communicate, to appropriate leader, all issues related to case escalation.
Demonstrates effective communication by being a critical link with attending and consulting physicians and all health care team members and payers. Facilitates resolution to any identified issues.

Performs other duties as assigned.
Required Qualifications:
Bachelor’s Degree in Nursing with 3 years clinical experience or AD degree with 5 years of clinical experience
3 years nursing experience in labor and delivery or post-partum or 5 years of clinical experience with ability to learn a new specialty area.
Active MN Registered Nurse license
Knowlegde of care coordination and experience with maternal/pediatric population required
Preferred Qualifications
Bachelor’s degree in nursing
5 years clinical experience in labor and delivery or postpartum care
3 years’ experience in care coordination in related specialty or public health

Have an understanding of hospital, community resources and resource/utilization management.
Have working knowledge of use of evidence- based guidelines.
Demonstrate critical thinking skills, problem-solving abilities, effective communication skills and time management skills.
Demonstrate ability to work effectively on an interdisciplinary team.
Have familiarity with computer systems and Microsoft applications.
Be available/able to work flexible hours

Compensation Disclaimer
The posted pay range is for a 40-hour workweek (1.0 FTE).
