JOB REQUIREMENTS: Clinical Pharmacist - Transplant - 31665 Work Schedule : 100% FTE, Day/Evening, 7:00am - 3:30pm/8:00am - 4:30pm, with rotating evening, 1:30pm - 10:00pm, and every 4 th weekend. Hours may vary based on department needs. You will work at University Hospital in Madison, WI. We are seeking a Clinical Pharmacist - Transplant to: * Provide medication therapy management services as part of the interdisciplinary transplant team to patients on inpatient and ambulatory services to assure that drug therapy is safe, effective, efficient, and cost-effective. * Perform both clinical and distributive tasks necessary to ensure that patients\' medication therapy is safe, effective, timely, and efficient. * Provide training and education to Pharmacy Residents and PharmD students through practical and didactic experiences. * Provide patient care to a diverse population including kidney, liver, pancreas, heart and lung transplant medical and surgical patients.
