MINIMUM QUALIFACTIONS
High School Diploma or equivalent
Valid Chauffeur license required and must have a safe driving record.
Must possess superior customer service skills and professionalism.
Must possess outstanding verbal and written communication skills
5.Able to transfer residents on and off bus and into appointments or other destinations as required.

- Consistently demonstrate (leads by example) Sanctuary Values to all internal and external customers (residents,
visitors, volunteers, and associates.) Create and maintain an atmosphere of warmth, propagating a calm
environment throughout the community.
Execute assignments in a culture that is shared and collaborative across all divisions within TSLC.
Follow established safety precautions when performing tasks and using equipment and supplies.
Ensure assigned work areas are maintained in a clean, safe, comfortable, sanitary and attractive manner. Ensure
established infection control and universal precautions practices are maintained when performing duties.
Assist with transportation of residents in a company vehicle to and from appointments, shopping, etc.
Provide and maintain the daily appointment log to be used by resident when requesting transportation.
Provide and maintain weekly sign up sheets for regularly scheduled runs (i.e., grocery store, malls, etc.).
Coordinate van schedule by reviewing individual appointments on a daily basis, scheduling departure with
residents.
Incorporate weekly runs into daily scheduled runs.
Coordinates information related to van schedule on an ongoing basis throughout the day with resident, facility
staff, office staff, medical offices, etc.
Advises supervisor and nursing staff of residents who exhibit difficulty in handling transportation responsibilities.
Accept full responsibility for physical welfare of residents while utilizing van services.
Maintains a monthly transportation schedule with Activity Directors to provide transportation to various
community functions.
- Provide assistance to those residents with limited physical abilities (i.e., help into van, fold up walkers and
wheelchairs, and load into van).
Responsible for preventative maintenance, refueling, and maintain overall cleanliness of the van on routine basis.
Maintains a log of van mileage on a daily basis.
Maintains a record of van usage, resident location, and individual destination.
Assist supervisor in planning, reviewing, and evaluating ways for improving the transportation system.
Coordinate appointment and transportation utilizing outside services as appropriate (wheelchair vans, ambulance,
public transportation, etc.)
- Coordinate staff to accompany residents needing assistance at doctor appointments or other appointments as
required.
