Job Description:

  • The Case Manager strives for excellence in direct patient care by building a collaborative caring work environment coordinating care and information across the various disciplines focused on patient outcomes and wellness.
  • The Case Manager assesses plans and facilitates options and services which best meet the patients' healthcare needs in the continuum of care.
  • The Case Manager formulates goals with the patient and their families.
  • The Case Manager advocates for the patient through communication and evaluation of available resources to promote quality cost-effective and safe outcomes.
  • The Case Manager incorporates standards of care for Case Management including identification/selection of patients for case management services, problem identification, planning, monitoring, evaluating, transition of care and outcomes.
  • The Case Manager implements practice based on current research and evidence based practice and ensures adherence to hospital and department policies, regulatory standards, and guidelines.
  • The Case Manager supports an environment of caring focused on the patient's healing and wellness.

Requirements:

  • Bachelor of Science in Nursing
  • Current RN in Kentucky
  • Basic Life Support
  • RN with five years' experience in acute care nursing, case management, utilization management or related field.
