POSITION SUMMARY: The Community Associate supports individuals by transporting and supporting them during medical appointments, ensuring necessary information is brought to the appointment and obtained from the appointment, and communicating updates with the assigned Care Coordination Associate (CCA) and Community Logistics Manager. The Community Associate is responsible for supporting assigned individuals with life activities as needed during the designated shift. DUTIES AND RESPONSIBILITIES Support individuals, based on their level and type of need as described in the Individual Support Plan (ISP), in going to medical appointments, understanding the reason for each appointment and bringing all necessary paperwork, insurance information, directions to the appointment location, money for parking, etc. Support individuals during medical appointments to communicate relevant information to their medical providers and obtain answers to questions Review Medical Appointment Summaries (MAS) and other documentation provided by the medical office prior to leaving the appointment; obtain additional information if further clarification is needed Review prescriptions ensuring the medical provider included a diagnosis and specific instructions; if medications are discontinued, obtain written documentation from the medical provider Support individuals to schedule a follow-up appointment as authorized by the CCA Obtain copies of all prescriptions and bring hard copies to the pharmacy if e-scripts cannot be sent. Inform the CCA and the Community Residential Services (CRS) team when the medication will be ready for pick up from the pharmacy Deliver all information to the CCA in the Logistics Coordination area Inform CCA and Nurse Case Manager if immediate follow up is needed after a medical appointment Review all assigned individuals' Individual Support Plans (ISPs) to be familiar with medical and any other special support needs of each individual Supervise, instruct, support and provide guidance to assigned persons served in accordance with their ISP Strictly follows physicians' orders for continued care Prepare for, and promptly respond to, emergency situations to ensure the safety and security of the individuals supported at all times, including but not limited to providing CPR, first aid and crisis intervention as needed in accordance with agency trained protocols. Complete paperwork including but not limited to the MAS and incident reports and maintain all documentation as required and as determined by their manager Ensure the cleanliness of Elwyn vehicles used by including but not limited to removing all trash, cleaning up spills and taking the vehicle to the car wash Assist in the resolution of problematic situations (i.e. communication difficulties, times of distress or agitation) that may occur for persons served; notify program management of any unusual situations, emergency situations or any problems unable to be resolved Notify proper authority in the event of an emergency and complete required documentation on the incident prior to end of shift Report all incidents as required by regulation and Elwyn policy Promptly report any infraction of Elwyn policies to program management Provide Community Participation Support (CPS) at Elwyn day programs as assigned Perform cardio pulmonary resuscitation (CPR), First Aid and crisis intervention techniques according to agency trained protocols Perform other duties as assigned IMMEDIATE SUPERVISOR: Community Logistics Manager DIRECT REPORTS: None CONTACTS:  Individuals, families, medical professionals, Elwyn employees EDUCATION/SKILLS/EXPERIENCE REQUIREMENTS: High School Diploma or equivalent One (1) year experience with individuals with intellectual/developmental disabilities Previous experience in residential and/or day programs preferred Demonstrate ability to work effectively as part of a team Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities Demonstrated strong attention to detail Demonstrated strong time management and organizational skills Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions Must possess excellent customer interaction, collaboration, and written and verbal communication skills Basic experience using Microsoft Outlook and Word Ability to perform cardio pulmonary resuscitation (CPR) and crisis intervention techniques according to agency trained protocols Current, valid driver's license in state of residence, three (3) years of driving experience in the United States, and acceptable driving record PHYSICAL DEMANDS/ENVIRONMENTAL PROFILE: See Physical Demands Checklist below which represents the general physical demands and environment conditions to which the employee holding this position must meet, in addition to the duties and responsibilities listed above, to successfully perform the essential functions of this job.
