Job Summary and Qualifications
We are seeking a Physical Therapist to join our healthcare team. In your role you will serve as an educator of patients and families. Evaluating all assigned patients and developing, implementing and modifying treatment plans in accordance with the patient’s need and physician direction. You will work with a phenomenal team that cares like family and works together in a collaborative manner.
In this role you will:
Evaluates assigned patients and establishes appropriate treatment goals and treatment programs.
Provides ongoing evaluation of patient’s status and goals, modifying treatment program for timely progression.
Coordinates treatment program in conjunction with other clinical/therapeutic services, as well as available community resources.
Provides for patient education to include the development of comprehensive and progressive home programs with instruction to the patient.
Job Requirements:
BSN in Rehab Therapy Masters or Doctorate preferred
Licensed Physical or Occupational Therapist in the State of Florida
BLS Required (American Heart Association)
One or more years of SNF/Acute clinical experience (Hospital Setting Preferred)
