JOB REQUIREMENTS
Education
Bachelor’s degree in social work or another related field or bachelor’s degree in Interdisciplinary Studies with at least 21 social work credit hours (LSW preferred).
Experience
Previous experience in community related programs preferred. Medical information knowledge required. Strong written and verbal communication skills.
Physical

Normal hospital environment. Hearing of normal and soft tones. Frequent sitting, standing, walking, climbing and reaching. Lifting and carrying up to 15 lbs. Uses computer, typewriter, fax machine and telephone.
JOB SUMMARY
The Recuperative Care Coordinator will carry out, in collaboration with other allied health professionals, the provision of services to individuals, groups, and community as required. They will be responsible for case management services for clients experiencing homelessness. They will refer to other agencies as needed. They will document and track grant funds and services provided to clients. The Recuperative Care Coordinator must interact with all other medical center departments, promote good public relations for the department and the medical center. This position requires a high level of organization and strong time management skills. Must have the ability to work effectively with people of diverse cultures and backgrounds. This position requires the ability to work independently.
