UPMC Pharmacist in Jamestown, New York

UPMC Chautauqua at WCA seeks Pharmacist in Jamestown, NY to fill prescription orders in accordance with State and Federal regulations while utilizing technological resources and equipment; communicate with physicians and nurses to ensure prompt clarification of orders that are unclear, illegible, or incomplete; identify and report suspected adverse drug reactions accurately and in a timely manner; interpret and evaluate physician orders to prepare, calculate, and dispense appropriate medication dosage forms for inpatient and outpatient treatment in accordance with the New York Pharmacy Practice Act, Federal Drug Laws, and other regulatory organizations; prepare and dispense intravenous admixture solutions, as required, using aseptic technique in an accepted sterile product preparation area; review, maintain, and screen patient medication records and profiles for allergies, drug interactions and therapeutic duplications while entering orders and/or verifying medication; sustain the formulary by minimizing non-formulary procurements, utilizing therapeutic interchange protocols, and promoting rational drug therapy selection; and adhere to all regulatory requirements, professional standards of practice, and hospital quality management policies and procedures in providing pharmaceutical care.

Job Responsibilities:

  • Communicate with physicians and nurses to ensure prompt clarification of orders that are unclear, illegible, or incomplete;

  • Identify and report suspected adverse drug reactions accurately and in a timely manner;

  • Interpret and evaluate physician orders to prepare, calculate, and dispense appropriate medication dosage forms for inpatient and outpatient treatment in accordance with the New York Pharmacy Practice Act, Federal Drug Laws, and other regulatory organizations;

  • Prepare and dispense intravenous admixture solutions, as required, using aseptic technique in an accepted sterile product preparation area;

  • Review, maintain, and screen patient medication records and profiles for allergies, drug interactions and therapeutic duplications while entering orders and/or verifying medication;

  • Sustain the formulary by minimizing non-formulary procurements, utilizing therapeutic interchange protocols, and promoting rational drug therapy selection; and

  • Adhere to all regulatory requirements, professional standards of practice, and hospital quality management policies and procedures in providing pharmaceutical care.

  • Must have at least a bachelor’s degree from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE),

  • Have a valid New York pharmacist license.
