The Agency for Persons with Disabilities works in partnership with local communities to support people with developmental disabilities in living, learning, and working in their communities. APD provides critical services and supports for customers with developmental disabilities to reach their full potential. The Agency serves people with spina bifida, autism, Down syndrome, children ages 3-5 at high risk of developmental disabilities, cerebral palsy, Prader-Willi syndrome, Phelan-McDermid syndrome, and intellectual disabilities. SPECIFIC DUTIES AND RESPONSIBILITIES
Employees of the Agency for Persons with Disabilities are required to demonstrate total commitment to outstanding customer service. This includes, but is not limited to, always acting in a responsive, professional, courteous manner with the employees, customers, and public we serve at all times.
This is a professional position responsible for helping individuals with disabilities in navigating the systems and provision of services, supports, and available resources to meet individualized needs, as well as identifying and address any barriers preventing individuals from accomplish their goals. The Disability Navigator is also responsible for connecting individuals to supports and services, in a timely manner, to address immediate or critical needs to stabilize the individual seeking assistance. The duties of this position include:
Conducting assessments to identify immediate needs, goals, and barriers. Formulating a care plan that clearly illustrates actionable steps and timeframes developed in partnership with the individual and need and caregiver if appropriate, to address areas of need and connect the individual with resources to achieve their immediate and long-term goals. The Disability Navigator shall provide the individual with referrals to local, community-based partners that can assist in providing services that address barriers and supports to help the individual achieve their pathway to possibilities.
When appropriate, providing connections to immediate supports in a timely manner to stabilize individuals and families and mitigate crisis situations.
Assisting individuals in accessing available programs, services, and optional methods for reaching goals. This includes providing warm referrals to other state agencies, local community-based providers or services, and resources to support individuals and their families.
Providing follow-up and monitoring of connections to service delivery and assessing outcomes and results.
Identifying and maintaining a working knowledge of available community resources to meet needs of individuals with disabilities and connecting individuals and families to relevant community resources and support.
Understanding the resources available from state government programs and connecting individuals to those services they need and qualify for.
Documenting all pertinent information and ensuring that forms are completed and uploaded into agency business systems accurately and timely. This includes preparing correspondence, case notes, narratives, technical reports, and other documentation.
Actively participating in trainings and team meetings on an ongoing basis.
Building and maintaining relationships with respect, trust, sensitivity, and confidentiality to the individual, coworkers, community, stakeholders, and partners.
Participating in required meetings regarding specific cases and attending other team or sister agency teaming meetings.
Perform other duties as assigned.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
Ability to : Understand, interpret, demonstrate, and manage oneself and to handle interpersonal relationships. Ability to listen and understand what others may be saying and asking questions. Ability to manage time effectively, prioritize tasks, set goals, and develop systems for achieving those goals. Ability to effectively communicate, respectfully conversing with others to achieve a desired outcome. Ability to write and edit using proper grammar and writing style outlined by agency and program requirements.
Demonstrate Skills in : Critical Thinking – analyzing available information, observations, and evidence in order to determine appropriate action.
Knowledge of : Must have knowledge and be proficient in-service orientation - actively looking for ways to help people and exceed the expectation of the person being served. Active Learning - Working with new material or information to grasp its implications and ability to apply.
Must have a High School Diploma or its equivalent.
Must have a valid driver’s license or other efficient means of transportation and be willing to travel for work related purposes.
Must have at least two years of experience in working with individuals with disabilities, care coordination, or in the health and human services field.

DIRECT DEPOSIT PROGRAM
As a condition of employment, a person appointed to a position in the State Personnel System is required to participate in the Direct Deposit Program. Rather than receiving a paper paycheck, your funds will be deposited directly into your account at your financial institution. This will be accomplished by Electronic Funds Transfer. Banks, savings and loan associations, and credit unions are eligible to accept such deposits. Retirement funds are also required to be in the Direct Deposit Program.
BACKGROUND SCREENING REQUIREMENT
It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.
No applicant for a designated position will be employed or permitted to volunteer until the Level 2 screening results are received, reviewed, and approved by the Agency.
Level 2 background screening shall include, but not be limited to, fingerprinting for Statewide criminal and juvenile records checks through the Florida Department of Law Enforcement, and Federal criminal records checks through the Federal Bureau of Investigation and may include local criminal records checks through local law enforcement agencies.
