Make a Difference
The Manager of Case Management provides onsite leadership to ensure that daily department operations function effectively and efficiently to meet the needs of all customer and regulatory groups. You will act as a consultant to ensure that the severity of illness and intensity of services interval criteria evaluations are completed on all patients as designated by Case Management goals. You will ensure that case managers assess, plan, and develop plans for patients based on recognized standards of practice for case management. You will support a culture of continuous quality improvement.
Exceptional Skills and Qualifications
Applicants for this position should be able to demonstrate competency as a Case Manager, exhibit team-building skills, and effect change by assuming leadership roles in interdisciplinary teams.
2 (two) year / Associate Degree in nursing from an accredited school of nursing is required.
Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana, as listed in the Nurse Licensure Compact (NLC) is required.
3 (three) or more years of clinical experience in a hospital setting is required.
2 (two) or more years of management experience is preferred.
Case management experience is preferred.
Understands the relationship of individual patients, units, and/or departments to the network, to facilitate the achievement of optimal goals with those relationships.
