Cleveland Clinic RN Homecare Visit Nurse in Independence, Ohio

Cleveland Clinic currently serves 13 counties in the greater Cleveland and Akron areas. Divided into 4 teams, the Homecare Nursing Program is assigned to patients within a designated geographical area. As an RN on the East team, you are responsible for delivering high quality support and medical services in a comfortable home setting within the Eastern Cuyahoga, Lake, Geauga and Ashtabula Counties. In this role, you will provide assessments, various treatments and education to help avoid readmissions. After seeing patients, you will complete documentation, make calls, attend meetings, coordinate additional care and arrange visit schedules. This position offers an excellent way to not only enhance the healthcare experience but also further develop your skills as a compassionate and dedicated nurse.

A caregiver in this position works days from 8:00AM - 5:00PM, with on-call, every fourth weekend and one winter/summer holiday requirements.

A caregiver who excels in this role will:

  • Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.

  • Participate in a team-based care model led by the Care Coordinator in each region.

  • Implement and monitor patient care plans established by, and report patient needs after each visit to, the Care Coordinator for further case management.

  • Perform accurate and complete starts of care, resumptions, recertification's and routine visits as requested and within specified time frames.

  • Perform medication reconciliation at each visit.

  • Assess patients for safety risks, abuse and neglect.

  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.

  • Report abnormal findings and/or changes in patient condition to the Physician and update Care Coordinators with those changes and any follow-up needed.

  • Communicate any patient's fall out/missed visits to the Care Coordinator immediately.

  • Participate in and support quality improvement projects and participate in department committees as assigned.

Minimum qualifications for the ideal future caregiver include:

  • Graduate from an accredited School of Professional Nursing

  • Current state licensure as a Registered Nurse (RN)

  • One year of current clinical experience as an RN

  • One year of experience in an acute medical/surgical or homecare setting

  • Valid Ohio driver's license and proof of $100,000/$300,000 automobile insurance coverage

Preferred qualifications for the ideal future caregiver include:

  • Bachelor's of Science in Nursing (BSN)

Physical Requirements:

  • Requires full range of motion, manual and finger dexterity and eye-hand coordination.

  • Requires standing and walking for extensive periods of time.

  • Requires corrected hearing and vision to normal range.

  • Requires some exposure to communicable disease or bodily fluids.

  • Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.

  • Physical Demand requirements are in excess of those for Light Work.

  • Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.

  • Must be able to read, write and communicate with patients.

Personal Protective Equipment:

  • Follows Standard Precautions using personal protective equipment as required.
