Houston Methodist Pharmacist Resident - Full Time - Days in Houston, Texas

At Houston Methodist, the Pharmacist Resident position is responsible for participating in an organized, directed, postgraduate training program that centers on development of the knowledge, attitudes, and skills needed to pursue rational drug therapy for a generalized populations as well as specialized diseases populations. This position performs fundamental clinical pharmacy duties and facilitates safe, efficacious, and pharmacoeconomically rational use of medications in the population(s) served. The Pharmacist Resident position supports the delivery of pharmacy managed protocols, formulary processes and medication reconciliation activities while providing education to fellow pharmacists, nursing, medical staffs, and others inside and outside the department. This position has additional pharmacy administrative, quality and research activities that are part of the training program requirements. A certificate of residency is granted to the Pharmacist Resident upon satisfactory completion of the residency program.

PEOPLE ESSENTIAL FUNCTIONS

  • Promotes a positive work environment and contributes to a dynamic, team focused work unit that actively helps one another to achieve optimal department results. Collaborates with other health care professionals as needed for problem resolution and/or achievement of identified patient specific goals.

  • Serves as preceptor and mentor for pharmacy students. Completes evaluation forms and assigns rotation schedules.

SERVICE ESSENTIAL FUNCTIONS

  • Provides fundamental pharmacotherapy management to patients. Activities include, but are not limited to, proactive drug therapy reviews and monitoring, providing drug information, patient counseling, and management of pharmacist-based dosing protocols.

  • Exercises good judgment in patient care decisions per performance standard and documents patient care activities and patient outcomes as impacted by pharmaceutical care and clinical interventions.

  • Serves as a guest or member on one or more Medical Staff committees and actively contributes to accomplishing the goals of the committee or subcommittee.

QUALITY/SAFETY ESSENTIAL FUNCTIONS

  • Collaborates with other health care professionals to manage patient's drug therapy for effectiveness, side effects, adverse drug reactions, toxicities, drug interactions, and incompatibilities following guidelines for formulary management and clinical intervention tools in the achievement of patient specific goals. Documents patient care activities and patient outcomes as impacted by pharmaceutical care and clinical interventions.

  • Completes rotation objectives and all assigned duties described in the identified specific residency program by demonstrating competencies that may include, but are not limited to, drug information and policy development, acute patient care, ambulatory patient care, research, project oversight, practice management, and weekend coverage.

  • Participates in departmental quality management activities, i.e. medication use evaluations, adverse drug reactions and medication error reporting, clinical interventions, medication center reviews, and inventory management documentation and reporting.

FINANCE ESSENTIAL FUNCTIONS

  • Contributes to the provision of pharmacoeconomically sound medication therapy by upholding HM Formulary standards in area(s) of responsibility.

  • Self-motivated to independently manage time effectively and prioritize tasks for self and for specialty area of focus.

GROWTH/INNOVATION ESSENTIAL FUNCTIONS

  • Conducts a major research project to advance the pharmacy department's goals and the pharmacy profession. Submits research and scholarly work for publication, platform presentations, and/or poster presentations. Gives presentations at local, state and national meetings, including in-services presentations to pharmacy, medical, nursing, and nutrition staff in area(s) of responsibility.

  • Participates in inter- and intra-department activities including in-services, article publication/publication, preceptorship, and additional training.

EDUCATION

  • Graduate from an accredited College of Pharmacy or recognition as a Foreign Pharmacist Equivalent Graduate by the Texas State Board of Pharmacy

  • Transcripts must be provided during the application process when requested

WORK EXPERIENCE

  • Zero experience; some experience in a hospital setting highly preferred

LICENSES AND CERTIFICATIONS - REQUIRED

  • PHRM - Pharmacist - State Licensure -- Texas Pharmacist license or eligible for Texas Pharmacist license within 90 days AND

  • BLS - Basic Life Support (AHA) within 90 days AND

  • ACLS - Advanced Cardiac Life Support (AHA) within 90 days AND

  • PALS - Pediatric Advanced Life Support (AHA) -- for pharmacists regularly assigned to respond to Code Blue events within 90 days AND

  • NRP - Neonatal Resuscitation -- if applicable to primary work assignment within 90 days AND

  • IV Certification - Intravenous certification (ACPE) -- if applicable to primary work assignment within 6 months

KNOWLEDGE, SKILLS, AND ABILITIES

  • Demonstrates the skills and competencies necessary to safely perform the assigned job, determined through on-going skills, competency assessments, and performance evaluations

  • Sufficient proficiency in speaking, reading, and writing the English language necessary to perform the essential functions of this job, especially with regard to activities impacting patient or employee safety or security

  • Ability to effectively communicate with patients, physicians, family members and co-workers in a manner consistent with a customer service focus and application of positive language principles

  • Exhibits strong interpersonal, teamwork and leadership skills with all levels of the healthcare team and assures delivery of excellent customer service to all patients, visitors, physicians and co-workers; capable of assuming responsibility for coordinating the activities of a major pharmacy service area which includes supervising and training peers, students, and supportive staff

  • Demonstrates the knowledge of basic principles regarding rational drug therapy, including but not limited to, professional and technical competence in the selection, evaluation, utilization, and distribution of drugs and drug information and apply these to all patient groups, neonate to geriatric

  • Experience with comprehensive pharmacy services including unit-dose drug distribution, intravenous admixture preparation, nutrition support services, and target drug monitoring

  • Competence in research in an area related to the practice of pharmacy and maintains active credentialing with HM Academic Institute as applicable to primary work assignment (refer to manager for details)
